We are seeking a dynamic and experienced Case Manager to join our Outreach Services Department.

 

What you will be doing: You will be responsible to identify, evaluate seniors' needs, and make quick and immediate judgement to determine and implement services to maintain or improve members quality of life.

  • Exercising independent judgment on matters of significance to 50 North such as program evaluation, expansion, training, and conflict management.
  • Assist the Director in creation, evaluation, and outcome processes for 50 North Outreach programs, preparing outcome reports with recommendations for modifications and changes.
  • Coordinate designated agency programs with changes, modifications and alterations as needed to conform such programs to meet the needs of the member.
  • Conduct member assessments in home or at 50 North including wellbeing, safety needs, limitations, and desires.
  • Advocate and coordinate on behalf of members to access necessary services.
  • Make judgments and decisions independently and with others regarding changes, modifications and/or alterations of the Outreach programs and processes.
  • Assist member when in a crisis situation.
  • Provide advocacy, linkage and collaboration with community resources based on client needs by problem solving and creating solutions.
  • Provide care coordination for contracted Senior Living Community.
  • Build rapport with seniors and families.
  • Collaborate with community resources based on senior needs.
  • Attend community meetings advocating on senior needs.
  • Keeps records and documentation in accordance with the agency's expectations, as well as the National Association of Social Workers.
  • Mandated reporter of senior abuse, neglect, and exploitation. Make reports to appropriate agencies as needed.
  • Coordinate services across 50 North departments, especially Chore and Nutrition Departments.
  • Complete Ohio Senior Health Insurance Information Program, (OSHIIP) training through the State Department of Insurance and yearly updates. Complete required documentation.

What you need:

  • Bachelor's degree in human services field; licensure through the Ohio Counselor, Social Worker, Marriage, and Family Therapist Board required.
  • 2 plus years' experience working with older adults or geriatrics preferred.
  • Strong communication, organizational and supervisory skills.
  • Knowledge of senior health and wellness issues and community resources.
  • Proficiency in computer applications (Excel, Word).
  • CPR, AED, and first aid certified or ability to obtain.
  • Valid driver's license.
  • Excellent verbal and written communication.
  • Ability to relate well to older adults and work effectively in a team.
  • Creative problem-solving and conflict management.
  • Ability to work flexible hours, including weekends as needed.
  • Multilingual in Spanish and/or other languages preferred.
  • Experience working with individuals from diverse cultural backgrounds preferred.
