Otter Tail County Social Worker - Vulnerable Adult Assessor in Fergus Falls, Minnesota

Otter Tail County is accepting applications for a full-time Vulnerable Adult Assessor Social Worker. The purpose of this position is to provide intake, assessment, and ongoing direct services and case management supervision for children and families in an identified domain of adult, children and family services, including developmental disabilities, long term care, foster care licensing, child care investigations and negative actions, mental illness, chemical dependency, vulnerable adult protection, child protection, child welfare, children s mental health, brain injury, adoption and minor parent. Workers establish collaborative relationships, participate and assist with the implementation of unit and agency goals, policies and procedures to provide optimal services to clients based on resources available to the agency. Examples of Duties Assess client health & safety within their environment based on social history presenting problem, observation and intake services. Determine immediate risks to health and safety and take appropriate action. Conduct formal assessments and screenings to collect, compile and analyze information to address client s desires and needs. Provide intake support for report of abuse/neglect including gathering information to facilitate an investigation and distribute to appropriate social workers, law enforcement, or other appropriate personnel. o Participate in VA screening reviews. o Consult with agency vulnerable adult screening team on all reports of suspected abuse and/or neglect regarding clients and assist investigation as determined necessary. Complete vulnerable adult assessment on screened-in MAARC (vulnerable adult reporting system) reports: o Review agency files for previous contact with vulnerable adult o Coordinate investigations with Law Enforcement and Public Health o Contact and interview the vulnerable adult to determine needs: ? Assess their general mental and physical health ? Assess their living conditions to determine safety and ability to meet their needs in the present environment o Contact and interview alleged perpetrator(s), along with witnesses to the incident and others who may have knowledge of the situation o Contact and interview collateral sources of information such as family, neighbors, friends, and agencies for background information o If the report involves a facility, review all internal records, charts & documentation o Offer assistance in obtaining services and/or discuss the options available to improve the vulnerable adults living situation o Prepare a report on interventions and actions taken during the assessment, and complete all mandatory documentation within timeframes required by rule or statute o Determine whether the report is substantiated, inconclusive, or false. Make a determination of maltreatment. o Seek emergency guardianship in cases of need for this service. Seek the establishment of a successor guardian or conservator in cases of abusive or neglectful Guardian or Conservator o Refer to County Attorney for possible criminal prosecution of the perpetrator Administrative tasks o Attend staff and supervisory meetings as scheduled o Attend conferences as scheduled or agreed upon by supervisor. o Attend training as related to job responsibilities. o Participate in public presentations regarding agency adult intake or CEP services. o Complete agency time sheets and employee records. o Utilize SSIS as the case management system, including timesheets, case notes, documents, and other systems as applicable. May assist with duties and responsibilities as outlined in the County Emergency Operations Plan and Continuity of Operations Plan. Perform other duties as assigned. Supplemental Information: This position is regularly scheduled to work 40 hours per week, and generally the work hours are Monday-Friday 8:00-4:30 p.m. The 2024 starting salary for this position is $32.30 per hour, depending on qualifications, with advancement to $51.23 per hour. Please note: This position is considered a safety-sensitive position due to the essential job duties in accordance with Minn. Stat. 181.950, Subd. 13. Safety-sensitive employees are subject to pre-employment drug testing and random drug and alcohol testing. In addition, all employees are subject to drug and alcohol testing upon reasonable suspicion/cause, return-to-duty, follow-up, and post-accident.
