Scope of Work

The Joint Replacement Coordinator plans, organizes and directs the general care of the orthopedic patient population in order to provide quality, cost-effective care. This position performs patient assessment and develops a care plan based upon appropriate input. Reviews, assess, monitors and assists in the evaluation of the pre-operative, operative, and post-operative care of orthopedic patients following pre-established criteria.Assures use of appropriate clinical pathways, external care, multidisciplinary collaboration; state and federal utilization management requirements as well as department and hospital policies and procedures. Collaborates with physicians, ancillary and clinical department staff, patients and their families regarding patient care. Intervenes to facilitate timely patient care services and treatments.Coordinates select continuing care services for patients (i.e. home health care, durable medical equipment, or extended care facility). Promotes maximum hospital reimbursement through coordination, preparation and analysis of diagnosis/census trends. Reviews and provides statistical information regarding variances and makes program care and quality improvement recommendations to department management.

Qualifications

  • Required Associate's Degree Requires successful completion of a Registered Nurse program from an accredited school.

  • 3 years of relevant experience- Requires a minimum of three years’ clinical experience coordinating patient care in either an acute care patient or physician office setting.

  • Requires a minimum one year of experience with orthopedic patients with demonstrated ability to anticipate and identify the multi-faceted needs of post-operative orthopedic patients.

  • Requires the demonstrated ability to incorporate the knowledge and skills of clinical practice while performing complex statistical analysis.

  • Registered Nurse (RN) - State of Michigan Upon Hire required
