Catholic Health Services Nurse Practitioner in Farmingdale, New York

Overview Community-based Palliative Care Nurse Practitioner JOB SUMMARY: The Palliative Medicine Nurse Practitioner works under the direction of the Chief Medical Officer for Hospice and Palliative Medicine. The Palliative Medicine Nurse Practitioner provides specialty level consultation and management to patients and their families facing chronic, complex and/or life-threatening conditions in the home setting and/or other community locations. Job Details The roles of the Advanced Practice Palliative Medicine Nurse include, but are not limited to: · Comprehensive medical and psychosocial assessments of patients and families with palliative care needs · Skilled and meticulous symptom management, including the prescription of pharmacologic agents and non-pharmacological therapies · Formulate, implement, monitor, evaluate and modify palliative care plans with input from interdisciplinary team members and other collaborating care providers · Serve as clinical and educational role model and mentor Education: Master's Degree in Nursing and graduate of an approved Nurse Practitioner Program required Experience and Special Skills: Previous experience in home-based services, complex cases and/or palliative care is preferred; some bedside hospital, oncology, ER or ICU experience a plus License and/or Registration: NY State RN license, Current NY State certification as a Nurse Practitioner with prescriptive privileges. Valid DEA registration number. Board Certification as Advanced Practice Hospice and Palliative Care Nurse (APHPCN) within one year of hire. Skills and Abilities: Excellent communication and interpersonal skills. Good verbal and written communication skills. Ability to perform diagnostic and treatment modalities required of specialty. Ability to interpret scientific studies required of the specialty. Ability to teach and counsel. Ability to work independently. Excellent critical analysis skills. Multi-task oriented. Strong critical thinking abilities. Ability to plan, organize, prioritize. Salary Range USD $130,000.00 - USD $165,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
