Overview Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home. Team 10 Covers Middle Western Suffolk ; Bayshore, Brentwood ,Brightwater, Deer Park, Dix Hills, Northern Babylon, West Babylon, West Islip, Wheatley Heights, and Wyandanch. Every Weekend - Saturday and Sunday 8am to 6:30PM Job Details Our Home Care LPN’s: Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. Measure vital signs, Administer IVs, change dressings, clean wounds as needed Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. Provide patient and family education for disease management and health promotion s directed by physician’s plan of treatment. Salary Range USD $32.00 - USD $40.00 /Yr. The pay for this position ranges from $32 – 40 per hour. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
