Kinship Navigator (Bilingual Spanish)

Job Summary:

You will provide information, referrals, wrap-around services, and/or legal guardianship services to kinship families to maintain residential permanency for the children.

What You Will Do:

• Conduct home visits, collect documentation, and complete written assessment

• Establish a trusting relationship with families through phone contact

• Provide to families with information and referral to accessible services

• Engage and provide support to families through the information and referral process

• Guide caregivers through the application process, including conducting an initial hierarchical needs assessment as well as providing additional information for referral to appropriate services

• Act as a liaison between caregivers and the program's contractor, the appropriate services providers

• Promote advocacy skills, empowering clients through appropriate program and service education, including

• effectively communicate with potential providers to ensure client's individualized needs are met

• Provide linkage referrals as needed

• Complete documentation within the program's allotted timeframe, as set by Program Manager and Program deliverables

What We Need to See:

• Bachelor's degree in social work or other behavioral health discipline from an accredited college or university

• Documented knowledge of one (1) year work experience with children and adolescent populations

• A valid driver's license and must be insurable by agency's automobile insurance
