St. Joseph Health Social Worker - Case Management in Eureka, California
We are looking for a Social Worker for the department of Case Management at St Joseph Health.
Location: St Joseph Hospital - Eureka, CA
Schedule: Full time, 80 hours biweekly. The work schedule will be Thursday-Tuesday.
Shift: 8-hour, Day - Variable
The aim of the Discharge Coordinator is to coordinate activities to facilitate efficient and effective patient flow across the continuum of care, including safe and timely discharge or transfer to an appropriate level of care.
Clinical expertise to ensure optimal assessment of needs.
Ability to develop and maintain collaborative working relationships with the care team, including but not limited to physicians, clinical staff, Outcomes Coordinators, Clinical Quality Coordinator, Medi-cal Liaison and community agencies.
Excellent interpersonal, verbal and written communication skills.
Excellent organizational, problem solving and time management skills.
Ability to compile, analyze and disseminate data.
Education: Master’s in Social Work is required.
Experience: Five years of experience as a social worker in a community-based setting, skilled nursing facility, or acute care facility preferred .
Knowledge/Training: Current working knowledge of state and national healthcare benefits for all age groups; community resources for adult and elderly patients. Understanding of eligibility requirements for state and federal medical programs for children, adults and the elderly (i.e.: CCS, Medicare, and Medi-Cal).
