We are looking for a RN Case Manager at St. Joseph Hospital!
Location: St. Joseph Hospital, Eureka, CA
Schedule: Full-time, 80 bi-weekly Hours
Shift: 8-hour, Day, 8:30 am to 5:00 pm
St Joseph Hospital is a nonprofit, 138-bed health care facility located in Eureka. This full-service hospital is now designated as a level III trauma center and has been a cornerstone of the community since 1920. St Joseph offers cardiac, critical care, emergency, maternity and infant care, neuroscience, oncology, orthopedic, surgery, and more.
The Registered Nurse (RN) Care Manager position required independent functioning, accurate assessment skills for acute care and discharge planning needs, and good communication skills. The Care Manager interacts with the nursing, medical, ancillary staff, patients, and patients’ families and significant others to assess the patient’s acute care needs, level of care, and discharge planning needs. The Care Manager certifies the acute stay, facilitates reviews for external review agencies, and issues denial letters for Intermediary of Medicare Quality Improvement Organization patients who do not require acute care. The Care Manager formulates, in coordination with others, the patient’s discharge plan, documents the discharge plan, and assists in the implementation of the discharge plan through appropriate referrals.
Ability to ensure patient confidence and comfort.
Ability to efficiently communicate with physicians, customers, and employees, both in person and by telephone.
Reads and comprehends written instructions; able to interpret medical orders, correspondence, memorandums, and medical records.
Applies deductive and common sense reasoning ability to problem solve on a frequent basis.
Initiative and the ability to make independent judgments are required.
Education: Graduate of an accredited nursing school and/or proof of
licensure by the state of California.
Experience: Minimum of two years of acute care experience. Experience in utilization of review process preferred.
Training/Knowledge: Working knowledge of state and national healthcare
