Qualifications:

1. Must be licensed with State of Kansas as an LPN or RN with a current certificate.

2. Must be in good standing with Kansas Department of Aging.

3. Must be able to perform job according to the physical specifications.

4. Must have a level of reasoning that allows the individual to make critical decisions.

Essential Tasks:

1. Upon start of shift complete assignment sheets for staff working that shift. Include breaks, meal assignments, weight, appointments for the day and all other shiftily task on the assignment sheet.

2. Receive shift report with off going nurse at the beginning of shift. Notify staff assigned to the shift of any information they may need in order to perform their job duties. Follow-up with the oncoming staff at 2pm and 10pm to update on needs to know information to care for assigned residents.

3. Obtain and document all skilled and Hospice assessments, under the assessment tabs, on assigned unit once a shift. Includes reason and diagnosis code for skilled stay and full assessment including vital signs with intervention put in place upon change in condition or abnormal assessment.

a. Residents will be charted on more frequently as condition begins to deteriorate during end of life. Goal is to keep resident comfortable and provide support to both resident and family.

4. Obtain assessment and chart on all open charts as indicated.

a. Review 24-hour report sheet and chart assessment as indicated on report sheet.

b. Chart and open resident chart on any change in condition that occurs during shift. Make sure to notify other nurses of change in condition on report sheet and during verbal report.

5. Monitor Blood Glucose levels in the absence of the float nurse.

a. Identify if BS is within range for individual. If out of range notify PCP according to policy

b. Follow hyper/hypo glycemic protocols for BS out of range

c. Administer insulin according to Physician orders.

d. Use appropriate technique for one handed safety method and disposal of sharps.

6. On days the treatment nursing is unable to complete treatments the charge nurse will perform all treatments as ordered by physician when indicated on treatment administration record.

7. Complete Admissions and Discharges per facility protocol. (Assessments, interviews, and ensure new admission have appropriate equipment, medications and treatments in place).

8. Process daily orders as they arrive.

a. Ensure all aspects of the order are completed

b. Ensure responsible party and pharmacy gets notified of changes.

c. All shifts must work together to process orders.

d. Night shift is to reconcile orders nightly to make sure adequate services and intervention for resident needs.

9. Monitor pharmacy fax tray and address outstanding pharmacy deliveries.

10. Assist family members, residents, and physicians with concerns in care or questions in regards to resident care. Initiate corrective action and report to DON.

a. Maintain compliance with HIPPA

11. Count and Verify PRN NARC box at beginning and end of shift to ensure proper management of narcotics in the absence of the float nurse.

12. Review lab orders, notify physician via fax or telephone (if critical results) of lab results. Process orders pertaining to specifics from physicians.

13. Draw labs as ordered when indicated if lab nurse is unable to obtain or SAT orders are needed.

14. Manage call-ins and assist in finding a replacement in order to meet resident needs.

15. Night shift specific tasks

a. Daily medication reconciliation of orders

b. Record refrigerator temperatures and implement interventions if needed for re dings outside range.

16. Interpret existing policies and procedures to nursing assistants, residents, families, and physicians.

17. Supervise, observe, and evaluate all direct resident care during job duties and initiate corrective action as necessary, notify DON when necessary.

18. Obtain and provide report from nurse he/she is relieving or coming on duty and record/provide sufficient information to implement appropriate follow-up action.

19. Assess, document and notify DPOA, physician, DON/ADON and Case managers of any changes in resident condition.

20. Identify safety hazards in the environment and initiate corrective action immediately including proper documentation where necessary.

21. Follow facility discharge/transfer procedure (including complete documentation) and coordinate discharge/transfer with other departments, physicians, and ancillary service providers.

22. Identify resident problems and emergency situations, and initiate immediate life saving measures within scope of practice in the absence
