Home Health Physical Therapist's and Assistants Home health PT's and PTAs take care for patients in their homes. They might see patients once a week or more depending on plan of care, the home health care therapist will act as a liason between doctors, nurses, patients, and families. Responsibilities: Includes vital signs monitoring, education and monitoring, assessments and evaluations, completing a plan of exercise, completing paperwork for billing, listens to patient's concerns turn in visit notes and met with all requirements to ensure that patients continue to receive appropriate services. Reports to Director of Nursing and maintain good communication with other therapists, nurses and office staff. Listens to the patient concerns and requests among other duties.
