Position Description:

Nephrology APP conduct comprehensive physical assessments and formulate plans of care. They order and interpret diagnostic tests. They treat chronic and acute health issues. Assess, diagnose, and manage systems-related complications of end-stage renal disease (ESRD) therapy including anemia, hypertension, bone disease, fluid and electrolyte imbalances, and vascular access. Diagnose and manage episodic health problems of clients with ESRD.

Major/Essential Functions:

• May review labs, medications, and health issues while completing a full examination. Health maintenance reminders.

• Responsibilities may included reviewing charts, seeing patients and families at clinic visits, writing orders and notes with independent billing, and reviewing post-visit labs with follow-up while keeping a consistent plan of care for optimal personalized CKD care

• Hospital follow-up visits always included a thorough review of inpatient records, including discharge summaries and a medication list.

• If patient medications were added or adjusted, make sure those changes were being implemented and followed by the patient.

• If patients were missing medications or did not understand the purpose for a medication, information was given, and medications were ordered or arranged to be ordered. Labs were reviewed and explained.

• Opportunity to impact dialysis regimen wellness, seeking input about management of care with the patient, which often leads to improved adherence, decreased acute issues, and ultimately less frequent hospitalizations.

• Rounds in the unit allow time to address and educate patients about alternate modalities of treatment, such as peritoneal and home hemodialysis.

• Review charts, including vitals, labs, medications, and trends, as well as any changes to the plan of care.

• May order recommended interventions, including dialysis treatments, labs, medications, or other changes to the plan of care, and communicate all these tasks with interdisciplinary team members.

• Round on assigned patients daily, provide updates on their plan of care emphasizing nephrology issues, conduct education daily, and document with consult or progress notes with billing components.

• Adhere to all TTUHSCEP policies, procedures and processes. Personally demonstrate| display and act in accordance with TTUHSCEP's values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork).

• Actively promote and encourage staff across the institution.

• Perform all other duties as assigned.

Required Qualifications:

• Graduation from a Physician Assistant's training program approved by the Council on Allied Health Education Accreditation of the American Medical Association

• Current Physician Assistant license in the State of Texas or ability to transfer licensee to Texas

• Certification as a Physician Assistant as evidenced by passing the examinations given by the National Commission on the Certification of Physician Assistants OR Graduation from an accredited school of nursing as a nurse practitioner from an approved certification program recognized by the Texas Board of Nursing

• Current R.N. license as approved by the Texas Board of Nursing or ability to transfer license to Texas. Certification as a Nurse Practitioner from an approved certification program as recognized by the Texas Board of Nursing or ability to transfer certification to Texas
