Job Overview:

Provides direct clinical social work/ licensed professional counselor/ licensed associate counselor services to patients and their families in a multidisciplinary team approach.

Qualifications:

Required:

• Graduation from an accredited graduate school of Social Work with an MSW (Master s in Social Work) degree. LSW or LCSW. NJ State licensure

OR

• Master s degree from an accredited school or counseling degree from an accredited school. LAC, or LPC. NJ State Licensure

• State licensure required by the NJ State board of social work examiners

• Licensed social worker (LSW) or licensed clinical social worker (LCSW) depending upon years of social work experience and clinical course work

• Licensed Associate Counselor or Licensed Professional Counselor

• Perinatal Mental Health Certification

• Successful completion of all orientation programs

Certifications and Licenses Required:

• Master s degree

• LSW, LCSW, LAC, OR LPC

• NJ State licensure

Scheduling Requirements:

• Shift working, 8:00am 4:30pm

• 3 days a week

• Part-Time

Essential Functions:

• Completes an initial psycho-social assessment including social work treatment plan

• Provides individual, family or group counseling, referrals to community agencies and entitlement programs

• Ensures continuity of care for patients' needs and maximizes the patient s and family s abilities to meet needs within their social and financial framework

• Understands stages of growth and development to assess each patient s age-specific needs to provide age-specific treatment and care

• Document interventions in patient s chart

• Other duties as assigned
