1. Make clinical judgments involving the observation, care and counsel of patients.
2. Administer medicines and treatments prescribed by the physician.
3. Make necessary changes to 485 Plan of Care prior to mailing (frequencies of all disciplines, teachings, orders, goals, medications, patient and physician demographic) once coder generates it from Axxess.
4. Submit Physician Order and 485 Plan of Care for MD signature. Keep track of all Orders and 485s and make certain MD signature is within Medicares 30-day period.
5. Track OASIS that are needed (Re-cert/Discharge/TIF/ROC) to ensure compliance with Medicares 30-day period.
6. Review RN/LPN/CNA/Therapy visit notes for completion prior to filing in the patients chart. Visits must follow the frequency indicated on the 485 Plan of Care. Verify that notes are answered completely with proper date and time of the visit and signature of the patient/PCG and visiting discipline.
7. Review the 60-Day Summary, Care Coordination, and Supervisory Visit of all disciplines.
8. Complete lab requisitions and keep track of results for submission to the ordering physician.
9. Assist/resolve field staff issues regarding patient care. Call patients to assure timely provision of nursing services, appropriate therapy, and DME.
10. Answer/resolve phone calls regarding patient care.
11. Follow-up with field staff for timely submission of notes and OASIS. Counsel field staff, if necessary, regarding frequency, OASIS accuracy, teachings, orders, etc.
12. Track patients admitted to the hospital and follow-up on discharge papers/orders upon their discharge.
13. Approve charts for billing once all notes are completed and posted in AXXESS before handing the chart to biller.
14. Document all correspondences with physicians and other disciplines in the patients chart.
15. Update medication profile every month and/or as needed.
