Director of Population Health

Position Summary:

The Director of Population Health is responsible for Griffin Health’s accountable care and value-based contract initiatives. The Director of Population Health will provide clinical expertise for specific, complex and/or rising-risk patient populations to help Griffin meet its accountable care quality targets and broader institutional goals. This includes establishing, meeting and continuously monitoring value based contract goals and objectives while the organization’s strategic goals and objectives are aligned with the department’s efforts. The position’s primary responsibility is directing the day-to-day operations, budgeting, financial management, team leadership/management and regulatory aspects of the Population Health Department. The Director promotes collaboration between the Population Health, Griffin Faculty Physician and Community Outreach teams.

Duties and Responsibilities:

  • Provides leadership and oversight to Population Health programs and teams

  • Develops population health initiatives to improve patient satisfaction and person-centered care

  • Builds interdepartmental and interdisciplinary relationships to assure collaboration and effective/efficient operations within the organization and in the community

  • Ensures the programs are aligned with the standards of the patient-centered medical home care model and fosters the administration of evidence-based healthcare in new and innovative ways

  • Identifies best practices for improving quality and lowering cost and incorporates them into Griffin’s Population Health model

  • Leads and/or serves on a variety of internal and external committees to represent Population Health

  • Reviews and analyzes population and patient data related to clinical care and outcomes to evaluate performance, reveal trends and identify opportunities

  • Prepares various reports to present findings and to highlight successes and opportunities for improvement or intervention

  • Formats and displays data in creative ways to effectively communicate findings to different audiences including healthcare providers, clinical team members and leadership

  • Establishes an annual operating and capital budget, demonstrating fiscal responsibility through meeting budget targets

  • Monitors and analyzes financial data and utilizes for decisions regarding staffing and operational budget

  • Researches grant opportunities that could provide additional financial support for Griffin Health’s population health priorities

  • Ensures completion of required grant reporting and contract compliance

  • Works to develop and implement performance improvement strategies to meet/exceed quality of care expectations

  • Engages staff and other stakeholders in continuous improvement of systems and processes

  • Facilitates assigned staff time and attendance to include requests for PTO and minimizing overtime

  • Oversees hiring, orientation, training and development of assigned team members

  • Provides coaching and counseling utilizing approved performance feedback tools and methods

  • Maintains a strong focus on ensuring that all work efforts are aligned with organizational goals and objectives

  • Enables staff to achieve optimal results by providing information, resources, encouragement and support

  • Works with other organizations - including the Value Care Alliance (VCA) – to meet and exceed the accountable care contract goals established by ACO payer partners

  • Represents Griffin Health on committees and workgroups convened by VCA and ACO payer partners to meet or exceed the organization’s accountable care contract goals

  • Other duties as assigned

Requirements:

  • Bachelor’s degree in a relevant subject area such as Public Health, Nursing, Health Administration, Social Work, or related field Required

  • Master’s degree in a relevant subject area such as Public Health, Health Administration is preferred

  • 3-5 years of relevant experience in the management of population health-related programs
