Matrix Providers is hiring a Registered Nurse (RN) Special Needs Coordinator to join our team of talented professionals who provide health care services to our Military Service Members and their families at the USAFA in Colorado Springs, CO.

  • Role: Registered Nurse (RN) Special Needs Coordinator (SNC)

  • Exceptional Family Member Program Medical (EFMP-M)

  • Compensation: $45-$50 per hour +$5000 Sign on Bonus

  • Schedule: Monday - Friday, 7:30 AM to 4:30 PM (40 hours per week)

Requirements: The Registered Nurse (RN)must have and meet the following:

  • Degree: Associate's Degree in Nursing.
  • Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN) and the Commission on Collegiate Nursing Education (CCNE).
  • Certifications in addition to Basic Life Support: Certified/certification eligible in relevant specialty, such as Certified Managed Care Nurse through the American Board of Managed Care Nurses or Certified Informatics Nursing, Ambulatory Care Nursing, Medical-Surgical Nursing or Nursing Case Management through the American Nurses Credentialing Ce Experience: Minimum of 3 years total nursing experience in a direct patient care clinical setting.

  • Must have nurse management or case management experience for 24 recent consecutive months.

  • Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse. Registered Nurse, Special Needs Coordinate Job Summary:

Clinical Assessment and Identification Procedures.

  • Utilizes clinical assessment skills to identify sponsors with special medical and educational needs promptly.
  • Maintains procedures for timely identification of sponsors' family members with special needs. Collaborative Approach and Outreach

  • Implements a multi-disciplinary approach in collaboration with key service providers.

  • Collaborates with installation youth and childcare facilities, officer and enlisted spouses groups for effective outreach. Enrollment and Data Management

  • Ensures appropriate "Q coding" (EFMP-enrollment) for family members of AF personnel.

  • Identifies sponsors with family members having special needs in AF medical special needs data management systems. Training and Consultation

  • Provides training to installation personnel and medical staff on EFMP-M implementation.

  • Offers consultation as needed to support EFMP-M implementation base-wide.
  • Family Member Relocation Clearance (FMRC) Integrally involved in the FMRC process for EFMP families.
  • Ensures FMRC requirements are met by EFMP-M staff. Oversight and Coordination

  • Provides oversight for base-level data entry in AF-provided data management systems.

  • Coordinates enrollment process and travel screening for active-duty family members. Maintenance of Special Needs Records

  • Ensures SN files for sponsors with family members with special needs are maintained at MTF.

  • Contacts Q-coded sponsors annually to assess unmet needs and updates information. Collaboration and Referral

  • Collaborates with Integrated Delivery System (IDS) and other interagency forums for medical information and referrals.

  • Refers EFMP families to Airman and Family Readiness Center for additional community assistance.
  • Actively supports integration of EFMP-M, EFMP-FS, and EFMP-A services at the installation.
  • Maintains cooperative working relationship with base MPS, CSS, and AFPC. Training and Development

  • Participates in clinic orientation and staff training for Special Needs Program.

  • May serve on committees, work groups, and task forces at the facility. Compliance, Training and Additional Skills
