UCHealth Pharmacist Clinical Outpatient in Denver, Colorado

Department: Pharmacy Atrium

FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks)

Shift: Rotating

Pay: $50.76 - $76.14 / hour. Pay is dependent on applicant's relevant experience

Summary:

Coordinates prescription monitoring programs and oversees comprehensive outpatient medication management to optimize patient medication use and adherence as part of the UCHealth Specialty Pharmacy Patient Management Program (PMP).

Responsibilities:

  • Tracks patient adherence to specialty medication regimen, provides counseling and clinical expertise on complex specialty disease states and monitor patient for side effects or other medication related problems.

  • Provides drug information and education to consumers. Communicates with physicians and other clinicians as necessary to facilitate care.

  • Upholds federal, state, accreditation, and payor requirements for the specialty pharmacy.

  • In conjunction with a patient's healthcare provider, provides, delegates, and coordinates patient care, which may include point of care testing and follow up care.

  • Develops and leads quality, safety, and affordability-related initiatives at clinic/unit, service line and department levels. Documents interventions and their outcomes for use in drug therapy quality improvement efforts. Participates in research and committees for the purpose of evaluating and advancing standards of care.

Requirements:

  • Doctor of Pharmacy from an accredited school of Pharmacy.

  • State licensure as a Pharmacist.

Preferred Experience:

  • 2 years specialty pharmacy experience OR PGY1 or clinical fellowship completion with 2 years' pertinent work experience OR Pertinent PGY2 completion.
