Job Summary

Serves as a patient advocate for all LGBTQ patrons looking for open affirming care and coordinates both primary care and specialty care appointments. Under general supervision, this position will navigate transgender patients through a surgical application process, ensure requirements are met for insurance coverage, schedule appointments, and educate patients on process and follow-up on patient experience. Case management of all surgical patients is required and will be documented in appropriate transgender surgical database

Essential Functions:

Serves as a patient advocate for all LGBTQ patrons looking for open affirming care and coordinates both primary care and specialty care appointments to ensure care quality and superior service (10%) Coordinates the pre surgery evaluation process and provides education and process education to transgender members seeking transgender surgery (10%) Works with patients to identify insurance coverage for procedures (10%) Coordinates services required to obtain approval for procedures as defined in policies/procedures; confirms that clinical requirements have been met, and coordinates the referral process as defined for potential surgical candidates (10%) Case manages all patients wanting gender confirmation surgeries (10%) Ensures communication between patient's primary doctors and surgical teams (10%) Collaborates on care and discharge planning issues, follows up with patient within a few days of discharge after surgery. (5%) Participates in designated transgender committees as assigned (5%) Participates in the development and initiation of policies and procedures, development of educational and operational programs and materials (5%) Collaborates with other teams to assure that all relevant data is being tracked and collected (5%) Identifies and reports issues of care quality, morbidity, mortality and service delivery as defined (5%) Maintains up to date information in transgender databases (5%) Provides amenity resources and information to parents, partners and family members (5%) Attends LGBTQ community events to help promote LGBTQ Center of Excellence (5%)

Education:

Bachelor's Degree Required

Work Experience:

1-3 years of experience in a clinic/hospital environment in a patient facing role Required and 1-3 years working directly with the LGBTQ community preferred

Knowledge, Skills and Abilities:

Basic care coordination Knowledge of community resources Establish and maintain rapport and a therapeutic relationship Communication and motivational interviewing Bilingual English/Spanish or English/other languages preferred Knowledge of Microsoft Outlook, Word, PowerPoint, Excel, and SharePoint Prior experience with an Electronic Medical Record system preferred, specifically EPIC Scheduling software experience preferred Ability to perform Web searches for clients.
