Job Summary

Under general supervision, delivers safe and competent occupational therapy as ordered by a physician. Plans, organizes and implements specific treatment programs or individual patients according to the principles and practices of occupational therapy. Develops, improves, or restores skills in activities of daily living, work, or functional activities. Directs and supervises work of support personnel.

Essential Functions: General Operations: * Performs occupational therapy treatment procedures and techniques in a timely, efficient and efficacious manner while utilizing support staff effectively and appropriately. Delegates patient care responsibilities per guidelines. Evaluates quality of care delegated to support personnel. Utilizes evidence-based occupational therapy treatment interventions. * Demonstrates and articulates sound clinical reasoning in synthesizing complex evaluation data identifying diagnosis and developing a comprehensive plan of care. Develops and update appropriate plan of care in collaboration with the patient/family and related to the patient's age and lifestyle with measurable long and short term goals and a defined D/C plan. * Demonstrates effective communication and interpersonal skills. Utilizes constructive communication skills for participation in: patient/family interactions, conferences, rounds, peer consultation, and operational communications. Conducts self in a professional manner in diverse environments and situations. * Completes patient and medical record documentation. Documentation is timely, pertinent, precise and efficient. Documentation meets agency and department standards. * Performs all required data collection for department and agency accurately and within designated time frame. Consistently meets designated patient care productivity standards. * Demonstrates competency skills in required areas of practice. Utilizes appropriate methods and materials for effective learning, monitoring response to education, and adapting program appropriately. Expands occupational therapy evidence and knowledge in practice. * Provides clinical education experience for students per request of supervisor or designee. Assists in orientation/training of new employees, OT students, hospital staff, and precepts new employees during orientation, as assigned. Trains and educates patients, family members and care-givers in therapy principles, methods and techniques in order to achieve intended outcomes. * Actively supports and participates in agency/department performance improvement and program development efforts. Contributes to overall quality, efficiency and productivity of work group and department. (20%) Departmental Duties: * Collaborates with appropriate parties to coordinate patient treatment authorization. * Updates and completes patient information per department guidelines to ensure timely and efficient transfer of patient care between care-givers. * Completes patient charges per department guidelines. * Achieves productivity standards. * Participates in performance improvement and quality enhancement projects. (20%) Evaluation: * Completes a thorough evaluation based on presenting diagnosis and pertinent information regarding the patient. * Initiates evaluation prior to treatment. * Completes appropriate data bases, problem lists, treatment goals and individualized plan of care * Treatment and goals are based on specific problems identified by evaluation. (20%) Treatment: * Clearly identifies treatment potential. * Sets goals to be o jective, measurable, reasonable, and described in functional or behavioral terms. A time frame for goal achievement is stated. * Involves patient and family/significant other in goal setting where appropriate. * Modifies treatment plan with re-evaluation or changes in patient status and coincides with goal and plan. * Ensures the treatment used is appropriate to the diagnosis and is modified to meet the changing needs or changing response to therapeutic intervention. (10%) Discharge: * Bases discharge on goal achievement, change in medical status, or departmental policy. * Provides discharge instructions to patient/family/significant other appropriate for age, and learning ability, need for adaptive equipment is identified and addressed. * Completes discharge summary and makes recommendations for follow-up are made. (10%) Communication: * Completes patient care documentation successfully. * Updates and completes patient documentation to reflect patient's current clinical status daily.
