Job Title: Lead Case Manager FLSA Status: Full Time - non-exempt Reports to: Program Manager Schedule: FT, varies Supervises: Case Managers Rate of Pay: $28/hour Close Date: 5/1/2024 Benefits: Standard; Full-Time, Exempt employees are eligible for but not limited to the following: Health, vision, dental, life as well as voluntary life and disability insurance Sick leave benefit - 1 day per month, 12 sick days per year (accrual and availability begins at hire) Vacation benefit - 20 vacation days per year, accrued at the rate of .0769 hours for each hour worked, excluding overtime (accrual begins at hire but may not be used until the completion of six months of employment) One floating day off for use (accrued immediately, and again annually, but may not be used until the completion of the initial three-month introductory period of employment) Pension Plan (after one year of continuous service) Voluntary Tax-Deferred Annuity Plan (403(b)plan) Function: The Lead Case Manager provides leadership to the housing focused case management services in Denver Metro Social Services. The lead case manager provides supervision of all case managers, housing navigators and interns in multiple sites across the Denver Metro area. This includes providing oversight for assessments, individual housing plans, and housing first program design for guests to successfully become rehoused in the broader community. Duties and Responsibilities: Oversee the provision of case management services and staff/interns Assess and develop a best practice re-integration program and case management plan for residents Develop employment training assistance for residents Network with and connect appropriate community resources/services Oversee Case Management Services and Staff Implement and oversee case management services that effectively transitions men to greater self-sufficiency Create a plan for utilizing internships within the case management program Supervise case management staff and interns within the program Manage a limited caseload of the most complex case management cases Establish standardized assessments and housing plans for each person within the case management program Oversee medical and oxygen room. Coordinate with local hospitals for guest discharge. Keep up-to-date documentation and records of all men participating in the case management program in appropriate databases. Produce timely data reports, including outcome measures and quarterly reports for city contracts. Monitor and track financials related to case management and housing services. Complete the following training: The Salvation Army Driver Training, The Salvation Army's Protecting the Mission Training, The Salvation Army Pathway of Hope Training, HIPPA Compliance Training, Clarity and WellSky databa Develop a Housing Focused Case Management Program Assess need and potential impact of a comprehensive re-integration program for single homeless men Create a case management program for shelter participants that assists and encourages them to work toward greater self-sufficiency. Assist in establishing a basic data analysis of program effectiveness and impact relative to outcome objectives and measures. Develop outreach and recruitment strategy to engage guests in case management programming. Housing First Employment Training Assistance Establish multiple and diverse employment training opportunities for program participants to reduce barriers to housing. Secure external career assessment and guidance services Promote community partnerships for permanent employment for residents Network with Community Resources and Partners Initiate networking relationships with appropriate community and service agencies. Encourage coordinated access/connection for residents with necessary community services Participate in community meetings and collaborations as assigned by the Shelter Administrator Promote communit and for-profit involvement with support Referrals as needed. Special Projects Offer leadership to special projects assigned by the Shelter Director or Denver Metro Social Services Director

Knowledge, Skills and Abilities Required: Commitment to the mission of The Salvation Army Strong communication and people skills Demonstrated history of providing professional, effective case management services Competent in providing a positive, solution-focused approach to working with troubled program participants Able to conceptualize, teach and model case management skills, including motivational interviewing and trauma informed care. Experience providing supervision to staff. Desire to serve others by building on their strengths Competent in working independently within clear parameters in a team environment

Qualification and Education Requirements: Master's in social work (M.S.W.) with at least 2 years, postgraduate, professional social service experience (L.C.S.W. preferred), preferred; or master's degree in social services field
