The role you’ll contribute:

The System Spine Program Manager facilitates an evidence-based, multidisciplinary approach to spine care throughout the care continuum at the campus. Provides direct clinical service to patients and families including care management and patient education to understand diagnosis, treatment and follow-up regimen. Provides education and mentoring to advance staff knowledge at the campus. Gathers, abstracts and reviews data for process and quality improvement. Help and oversee the navigation of patients/families through the medical system and ensures timely and appropriate evaluation, treatment, educational services; and access to appropriate resources, as needed. May facilitate/ coordinate patient support and/or educational groups and continually strive for innovative and creative strategies to impact and improve patient quality of life. They are responsible for the overall quality and functioning of programs related to spine care services.

The value you’ll bring to the team:

· Formulates an individualized care plan based on therapeutic principles as appropriate for age/psychological development; evaluates the effectiveness of nursing/medical interventions and makes modifications as needed in accomplishing desires patient outcomes; assists in the assessment of the patient with TeleMedicine as needed.

· Initiates patient coordination at the patient’s entry point into the system. Prepares patient case for pre-procedural review with multi-disciplinary team. Ensures patient case follows clinical path prescribed/therapeutic interventions via case review process.

· Navigates appropriate spine care inpatients from the admission diagnostic phase through the treatment and discharge phase, ensuring appropriate/timely medical/supportive services; good organizational skills in planning/implementing daily activities, and facilities early discharge of patients.

· Communicates effectively with physicians, healthcare team, patients and families to ensure coordinated and cohesive execution of evidence-based quality patient care; establishes/maintains a therapeutic relationship with patients/families.

· Serves as patient and staff resource and advocate, promoting access to available services, tools and support.

The expertise and experiences you’ll need to succeed :

• Minimum qualifications :

o Advanced knowledge of adult neuro and ortho spine related treatment options

o Skilled in providing age appropriate education and patient care by interacting positively with patients, families, staff, physicians and the community

o Strong communication and collaboration skills

o Bachelor’s degree in Nursing or Health Administration

o Three to five years’ experience in neuroscience nursing or related healthcare field

o Current valid State of Florida license as a registered professional nurse

• Preferred qualifications:

o Certification in Neuroscience Nursing (CNRN), Critical Care Nursing (CCRN)
