Geisinger Registered Nurse (RN) Patient Placement in Danville, Pennsylvania

Job Summary

Works in collaboration with referring and accepting physicians to facilitate effective and efficient transfers of patients between health care facilities. Works with attending physicians and inpatient unit charge nurses to facilitate timely and appropriate bed placement in accordance with established level of care and patient aggregation criteria.

Job Duties

  • Manages the inpatient flow of patients from preadmission and admission, through the time of discharge and efficiently optimizes the utilization of inpatient bedsdentifies demand and capacity mismatch for inpatient beds.

  • Works in conjunction with referring physicians and nurses, accepting physician and nurses, and other interdisciplinary team members to assess, plan, and facilitate inpatient transfers.

  • Provides clinical screening on all admission requests using Interqual Criteria for medical necessity.

  • Screening is done for appropriateness and pre-certification.

  • Collaborates with the staff in utilization mnagement for follow up on urgent and emergent transfers and admissions.

  • Documents communication note in electronic medical record, with detailed inpatient, clinical information including reason for transfer and details of transfer plan.

  • Facilitates and coordinates the details of admissions with departments.

  • Coordinates inpatient bed assignments based on acuity, aggregation and bed availability.

  • Communicates to appropriate attending physician for issue resolution or intervention.

  • Monitors process of admission and mitigates any delays.

  • Identifies any quality management issues and reports to appropriate manager for follow-up.

  • Serves as a resource to physicians and hospital personnel regarding system-wide, inpatient bed availability.

  • Directs and alters assignments in response to changes in capacity and acuity of department.

  • Participates in coverage and on call systems per department policy.

Position Details

About Patient Placement:

The Placement Services team works behind the scenes to make the admissions process go as smoothly as possible. Twenty-four hours a day, seven days a week, the team coordinates admissions for our patients in need of hospital care. You'll partner closely with our Physicians, Emergency Departments, Clinics and more to schedule patients for appropriate placements within our hospitals. The Placement Service's team uses diagnostic, triage and other information they collect through their conversations with physicians, nurses and medical staff to create a plan of care that determines where the patient should be assigned.

  • Hours: Evening Rotation: Hours vary between 7a-7p, 7a-3p, 11a-7p, 9a-9p, 9a-5p, 11a-11p

  • Travel: None

  • Weekends: Every other weekend

  • Holidays: Every other holiday

  • Required experience: 3 years of RN work experience

  • On-site orientation in Danville. Orientation can be several months long.

  • After completing orientation, work from home may be permitted if you're in good HR standing and meeting department metrics.
