Geisinger Program Director Clinical Outcomes Redesign (RN) in Danville, Pennsylvania

Job Summary

Functions in a leadership role to manage the clinical redesign team oversee complex projects, facilitate change, drive substantial improvements in quality and efficiency, and share accountability with clinical and operational leaders in achieving transformational results at both an internal and external program level.

Job Duties

  • Develops work plans for discrete projects and multi-project initiatives.

  • Manages the work to meet project milestones through task management, consistent follow through, relationship management and creative organizational problem solving.

  • Consistently achieves project outcomes at target goals and reaches stretch goals frequently through consistent follow-through, creative problem solving, removing barriers and tenacious focus on results.

  • Hardwires process improvements and embeds process and outcome metrics in improvement initiatives that draw data electronically from clinical and administrative records; is presented in easy to understand graphic format; and has sufficient detail to identify specific sources of actionable opportunity.

  • Formulates clinical improvement strategies and approaches across the continuum of care, designs complex improvements in patient care workflow through multiple settings of care and leverages clinical experience to solve difficult clinical issues.

  • Through Process and Relationship Management, assesses the local stakeholder environment and builds the support and will to address improvement opportunities among line staff, physicians, clinical and operational leaders as well as community-based program leaders.

  • Establishes and manages relationships to achieve transformational change with line staff, department managers, vice president, physicians, physician program leaders and physician chairs.

  • Facilitates and trains interdisciplinary committees, and is able to support manager and physician leaders in running interdisciplinary committees.

  • Uses analytic and subjective assessment skills to identify substantial performance improvement opportunities for important clinical, operational or financial issues.

  • Identifies and applies relevant best practice improvement solutions from prior training and experience, internal best practices, literature and external relationships and applies them to current projects; consistently innovates on best practice approaches in context; becomes industry thought leader in focused areas.

  • Collaborates with others both inside and outside of the organization to utilize expertise from multiple settings of care.

Education

Bachelor's Degree- (Required), Master's Degree- (Preferred)

Experience

Minimum of 5 years-Managing people, processes, or projects (Required), Minimum of 5 years-Nursing (Required)

Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania
