Our Cumming office services home health patients in their private homes and assisted living facilities throughout Cherokee and Forsyth Counties as well as Alpharetta, Johns Creek, Buford, Sugar Hill, and Lawrenceville/Gwinnett. Again, you can pick your work schedule and service area. The only LPN requirement for this position is a GA LPN licenses and the desire to work in a home health setting. You bring the passion for home health, and we will supply the rest! Not necessarily looking for a full-time opportunity? We are always hiring quality PRN and part time LPN's.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan

• Complete all documentation and records in a timely and accurate manner

• Follow established policies and procedures as related to the scope of practice

• Maintain continuity of patient care in a home care setting

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice
