What you'll do Under general supervision of the Clinical Care Coordinator (C3), the Unit Continuing Care Assistant (Unit C3a) provides non-clinical administrative support for continuing care unit activities. The Assistant supports unit staff with ensuring Care Logistics software accurately reflects current unit status. Reviews records, documents, and plans; collects and reports information Uses data collection tools to reflect activities, as noted by review of documentation Provides community resource information as requested and facilitates or completes referrals as directed Assures appropriate classification is complete for each patient, each shift Reviews Physician admission order and validates status and level of care matches order Updates bedded patient status. Ex: Observation to Inpatient; Inpatient to Observation when needed Identifies potential discharges and updates patient status and planned discharge time in Care Logistics software Assists Unit C3 and primary Nurse in ensuring all patient information in the Care Logistics software and documentation software is up to date Assists C3s with referrals; faxes and photocopies medical record information as needed for post-acute patient placement to facilitate timely discharges Support Unit C3s and Complex C3s with setting up transport, DME, and Follow-up appointments Collects patient’s insurance authorization for medications and discharge needs Notifies payer, as required, of patient’s admission and level of care —either online or via telephone Assists in the submission of medical information to payers and community care providers as needed Receives referrals from Unit C3 for discharge planning assistance Assists Complex C3 's with difficult discharge planning documents (faxing, calls etc.) Follow up with post-acute care facilities on bed holds Sends out faxes and clinical information as assigned for referrals to the following community, SNF, Rehab, tribute, wound care, and other post-acute LOC as assigned by Unit C3 Obtain necessary medical records and documentation Crosstrain to provide support for the logistics transportation Assesses payment source, determines coverage for needed services, including obtaining authorization (medication, STR, transportation etc.), in advance of discharge implementation. Completes referral for the Health Insurance Eligibility Specialist and follows up as needed. Verifies patient’s insurance eligibility for direct admits and facility transfers; obtains authorization if necessary Updates/corrects insurance information; notifies departments to ensure accurate financial data is available. Collaboration Keeps Unit CCM informed of status, interventions, and plans to progress patient care to discharge Actively communicates with multidisciplinary healthcare team (Primary Nurse, nurse manager, C3, etc.) Meets with Unit C3 and Complex C3 daily to identify patients with financial assistance needs and community service needs (CHHA, meals on wheels, lifeline etc.). When directed by Unit C3 and/or Complex C3, initiates and completes referral(s) or any other Community Services Referral Attends SNAP rounds on acute care units report progress with discharge plan details Meets with patients to provide information and discuss services being arranged (CHHA, DME. Infusion, wound vac etc.). Communicates discharge plan details to patient and family as needed Visits the patients to assure patient’s choice of agencies or preferred provider has been selected. Provides patient or patient representative with other service options available in their area. Visits patient or patient representative to obtain required information to initiate referral(s). Reviews financial obligation with patient or patient representative as related to referral. Contacts referral destination in person, by phone, fax or EPIC to notify of early alert and final alert. Performs Other Duties What we need from you Education/Qualifications: High school diploma or GED preferred Experience: 2 years of experience working with regulatory programs, administrative/secretarial in a professional setting, or patient scheduling/registration experience required Associates degree or higher will waive the experience requirement Skills: Communication Build Relationships Computer Skills Multi-Tasking Attention to Detail Self-Motivated Office Administration Physical Requirements: Sedentary Work - Exerting up to 10 lbs of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking or standing are required only occasionally, and all other sedentary criteria are met. Pay Range Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement. Pay Range Minimum: USD $16.65/Hr. Pay Range Maximum: USD $24.97/Hr.
