Gurwin Jewish Nursing & Rehabilitation Center Medicaid Coordinator in Commack, New York

Gurwin Jewish Nursing & Rehabilitation Center is a renowned healthcare provider located on Long Island, NY which offers a full continuum of services. The Nursing & Rehabilitation Center is a 460 bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, an adult day health program, ventilator dependent care, palliative and hospice care, a Memory Care Unit and on-site dialysis. The Gurwin Family of Services also includes the 201-apartment Gurwin Jewish Fay J. Lindner assisted living community and the Fountaingate Gardens independent living community on the Commack campus.

EDUCATION/EXPERIENCE REQUIREMENTS INCLUDE: High School Diploma, minimum 2 years SNF, DSS or related experience.

JOB DESCRIPTION:

Meet with families of prospective residents prior to admission. Collect and review all documentation required to file Medicaid application. Advise prospective resident family members regarding Medicare and Medicaid regulations, facility policy, and to answer all questions. Investigate and process Medicaid Conversions, Recertification’s, Applications, Provider Changes, and monthly Roster’s for all counties.

RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:

  • Conduct pre-admission interviews with prospective resident family members, collect all documentation required for Medicaid application, review all facility financial policies, have all required financial agreements signed, and explain Medicare and Medicaid regulations that apply to prospective resident family members.

  • Review all documentation including 60 months of banking to determine Medicaid eligibility or penalty periods incurred.

  • Contact case workers at hospital, Medicaid, or attorney to follow up on Medicaid applications.

  • Keep chronological listing of all contacts and follow-up calls.

  • Process Medicaid Applications as assigned with residents, families, attorneys and respective Medicaid office in a timely and professional manner.

  • Review Admission Logs to identify new Medicaid Pending Applicants on a timely basis.

  • Prepare and process Medicaid Pick-Up Statements efficiently and accurately.

  • Process new, revised, and corrected Pick-Up dates when needed, and follow-up with Attorney’s Case Workers and Elder Care Agencies to notify them of any changes on a timely basis.

  • Conduct Face-to-Face interviews with Family and representatives while upholding a hi-level of professionalism and efficiency.

  • Process recertifications for appropriate county and follow-up with residents and families in a timely and professional manner.

  • Process new, corrected and revised budgets in a timely and accurate manner with related parties.
