*TITLE: * Spoke Care Navigator -- 0.5 FTE

DUTIES:  Identify patients with opiate use disorders who are in need of Medication Assisted Treatment (MAT) to facilitate a smooth transition between NEWACS (Spoke) and Providence (HUB) to ensure that the patient is successful in the MAT process.  Refer at least 2.5 MAT patients per month to the HUB for MAT services.  Conduct patient screenings, assessments and evaluations to include initial assessment of social, medical, mental health, substance abuse and medication history.  Patient education of MAT, assist in monitoring of patients, coordinate referral for MAT, completion of required clinical paperwork required by the HUB, HCA and to DSHS, and work collaboratively with the HUB's staff.

*MINIMUM REQUIREMENTS: * Must have lived experience, defined as a past recipient of substance use disorder treatment services, and be in active recovery for a minimum of 12 full months, and able to provide appropriate Peer Support Services without impairing one's ability to remain in ongoing, stable recovery.  Must obtain state certification as a Certified Peer Counselor and Agency Affiliated Counselor-registration within 6-months of hire.  High school graduate or GED required. Must have good written & oral communication skills and be able to complete administrative & clinical records. Computer literacy required -- clinical software used. Abstract of driving record for past three years required. Satisfactory criminal history backgrounds check results are a condition of employment.

SALARY AND BENEFITS:  $25.17 - $33.79/hour based on qualifications and experience.  This position is part-time/non-benefited.
