Catholic Health Initiatives Clinical Pharmacist in College Station, Texas

Overview

Oversees all medication storage and preparation St. Joseph Health System. Ensures that the pharmacy provides optimal services, meets all legal, accreditation, and certification requirements, and complies with all applicable policies, procedures, codes, and standards for the organization.

Responsibilities

  • Fulfills licensure and thus fulfills continuing educations requirements set by TSBP as well as all competency training provided at St Joseph Health

  • Works closely with Director of Pharmacy, Pharmacy Operations Manager and Clinical Pharmacy Manager as required to fulfill job responsibilities

  • Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.

  • Contributes to the quality and effective operation of the department by supervising pharmacy technicians

  • Integrates the department into the organization`s primary functions by abiding to quality improvement programs, medication utilization evaluation, Pharmacy and Therapeutic Committee decisions, Pharmacy Policy and Procedures, and Multidisciplinary Policies and Procedures.

  • Does all job descriptions of a staff pharmacist such as entering medication orders, verifying medication orders, overseeing medication disbursement by technicians, communicating with nursing staff and medical staff concerning the safety of our patients.

  • Communicates both verbally and in writing in a professional manner to all disciplines within the organization to establish positive and productive relationships.

  • Maintains current knowledge of all drugs to be the Drug Information Specialist for all disciplines in the hospital as well as for the patients.

  • Understand how to access and operate any Automated Dispensing Machines, UD pre-packaging, or any other equipment in the pharmacy.

  • Actively participate in any committee’s responsible for establishing and implementing medication-related policies and procedures as well as those committees responsible for the provision of patient care as required by department leadership, including the P&T, infection prevention and control, patient care, medication-use evaluation, medication safety, nutrition, and pain management committees (or their equivalents), as well as the institutional review board, quality-improvement, and information technology committees (or their equivalents).

  • Performs drug utilization reviews to determine safe and appropriate use when verifying medications and completes responsibilities as defined by the clinical assignments document:

  • Pharmacy Consults

  • Antimicrobial Stewardship

  • Anticoagulation

  • Drug Use Evaluation

  • Discharge Counseling

  • Multidisciplinary Rounds

Qualifications

Required Education and Experience Education:

  • Bachelor’s Science in Pharmacy from ACPE accredited School of Pharmacy

Required Licensure and Certifications

  • Licensed Pharmacist

Pay Range

$46.74 - $67.77 /hour
