Our trauma-informed system of care model focuses on increasing the self-sufficiency of the individuals and families, helping them to live their best life. THE IMPACT OF A LIFE COACH As a Life Coach, you will play a crucial role in providing advocacy, case management, and crisis intervention services to support young women in need. Your responsibilities will include assisting tenants in developing individualized service plans, conducting self-sufficiency assessments with all tenants to develop appropriate housing related activities and support services and facilitating life-skills groups and independent living skills sessions related to housing, as well as offering short-term crisis intervention services. You'll also maintain current client and program records, case notes, and other documentation in accordance to the standards of the NASW, develop relationships with community partners, attend multi-disciplinary team meetings as required, collect data for and produce outcome measurement reports. Working closely with the Clinical Director, you will develop crisis management plans and collect data to produce outcome measurement reports. By leveraging your problem-solving skills and empathetic approach, you will have the opportunity to make a meaningful impact in the lives of those you serve. WHAT WE'RE LOOKING FOR IN AN INDEPENCE PLACE LIFE COACH Successful candidates who demonstrate a willingness to work flexible hours to accommodate tenant needs and a commitment to ongoing education to stay current on topics relevant to the client population served. Knowledge and skills required for the position are: • Bachelor's degree in social work or a related field along with state licensure (Licensed Social Worker, Licensed Professional Counselor, Licensed Independent Social Worker or Licensed Professional Clinical Counselor) is preferred. • At least 2 years' experience working with and knowledge relevant to client population specific to program including teaching or group facilitation. • Knowledge of human development specific to program such as indicators of child abuse domestic violence substance abuses mental health diagnoses and suicide risk, and lethality assessment is required. • Must have a valid Ohio driver's license with proof of current auto insurance and a reliable vehicle. • Demonstrated excellent oral and written communication skills • Knowledge of foster care/child welfare system across Cuyahoga County and state of Ohio. • Willingness to work non-traditional hours (as needed) to address special circumstances of tenants. • Willing to remain current regarding social topics relevant to the client population specific to the program.
