Applewood Centers Clinical Diversion Program Manager in Cleveland, Ohio

Applewood Centers is a private, nonprofit agency providing mental health services, social services and education to children, adolescents and families living in Cuyahoga and Lorain counties. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.

Position Summary: The Clinical Diversion Program Manager’s primary function is to oversee the following programs: the Cuyahoga County Juvenile Court Early Intervention and Diversion Center (EIDC), Community Based Intervention Center (CBIC) and Domestic Violence Diversion Program (CALM). In this capacity, the Program Manager will ensure the appropriate placement of youth throughout our array of referral and treatment services.  The Clinical Diversion Manager will work with entities in our community to secure referrals to our programs and to promote Applewood services, including government, educational, legal, and direct consumers and their families. Further, the Program Manager will oversee staff in various programs that are assessing youth involved with Cuyahoga County Juvenile Justice system, developing safety plans when necessary and ensuring the proper placement of youth throughout our array of treatment services, or referring to services within other agencies, if appropriate. The Program Manager will provide guidance, instruction and direct supervision to these assessment team members.

Responsibilities Include:

  • Provide clinical and administrative supervision of EIDC, CBIC and CALM team members.

  • Assist with program on-call coverage.

  • Participate in program administration and clinical treatment teams, including internal and external agencies.

  • Administer, track, and submit required outcome measures in accordance with program standards.

  • Responsible for communication and service coordination with contracted providers.

  • Ensure proper and necessary staffing for the 24/7 triage location designed to meet the needs of the EIDC and CALM youth. Be available as backup when necessary to ensure team member support.

  • Assist Juvenile Court staff in ensuring successful follow up and linkage for youth and families.

  • Educate and train police, community agency, Applewood staff, and Juvenile Court staff on EIDC, CBIC and CALM services and procedures. 

  • Provide information on services to outside agencies, referral sources, and managed care companies across the county, as well as other forms of current and prospective purchasers of services, including educational partners, families, their guardians, attorneys, and representatives.

  • Maintain contact with key persons from various agencies through face-to-face meetings, phone, mail, e-mail, or other electronic media, in order to keep agencies informed of available services and to maintain awareness of referral source needs.

  • Plan, strategize, identify, and report trends and service needs from and about prospective customers.

  • Develop and maintain referral source database and waitlist.

  • Responsible for oversight of program staffing and shift coverage.

Qualifications:

  • ​​​​​Minimum Master's Degree in Social Work, Counseling, or Marriage and Family Therapy required.

  • Valid Ohio LSW, LPC, or MFT required.

  • 3 years’ experience in clinical diagnosis and assessment required.

  • 3-5 years’ experience working in the mental health field required.

  • Experience working with children, adolescents and their families.
