• 3 to 5 years of experience acute care inpatient hospital setting
• Graduate from an accredited school of nursing
• Associate's Degree, minimum required
• Bachelor of Science in Nursing, preferred
• Current OH RN License
• BLS/ACLS/PALS per department requirements based on patient population needs.

Monitors patients for discharge readiness and appropriateness to wait for ride home in Discharge Hospitality Center. Completes Discharge paperwork in Electronic Medical Record. Removes medical devices such as IVs and telemetry monitoring equipment and completes education with patients on inpatient units. Collaborates with Case Managers, Physicians, direct care Nurses and other care providers to ensure patient safety and understanding of discharge instructions and times. Proactively identifies potential barriers to throughput and discharge. Facilitates and conducts post-discharge outreach communications with UC Health patients, ensuring appropriate follow- up actions are taken to meet the identified needs of the patient/family.
