Description
Job Summary:
Work as part of a low vision team including a consulting optometrist and Certified Low Vision Therapists (CLVT), to provide low vision services, including developing a plan of care designed to help low vision clients reach optimal goal achievement.
Duties/Responsibilities:
Provide initial Low Vision Evaluations to address clients identified goals and initiate training with instruction in use of low vision optical and non-optical vision aids, ADL/IADL training within CABVI guidelines, compensatory strategies, and adapted techniques, complete referrals in the best interest of the client.
Complete all paperwork, Evaluation, Progress Note, Discharge Summary, Superbill or other Billing Sheets, Equipment Loan Agreements, Inventory tracking, Mileage and Day sheets with accuracy and within stated time frame.
Under the direction of the optometrist, provide appropriate therapy and follow-up training in the agency and in the home.
Educate patients, family, caregiver on low vision services and resources both within CABVI and in the community at large.
Provide service at Gilbert Avenue low vision clinic and in mobile or satellite clinics as needed.
Collaborate with other specialists, including Vision Rehabilitation Therapists, Access Technology Specialists, Social Workers, Orientation and Mobility Specialists and other CABVI professionals to provide client solutions based on specific needs.
Complete special projects, marketing, or other assignments to help meet program or agency needs.
Actively participate in program planning, program development and identification of unmet needs.
Fully prepared for talks/presentations/projects; projects meet goals and are completed within a timeframe.
Qualifications
Education and Experience:
Licensed Occupational Therapy, OTR/L and certification in Low Vision (SCLV) through the American Occupational Therapy Association or completion of the Low Vision Certificate Program and/or ACVREP CLVT certification are a plus.
NCBOT certification.
Licensed in the state of Ohio as an OT; (Ky licensure a plus).
Have (2-5) years of experience in low vision, geriatric, or neuro rehabilitation settings.
Have a good understanding of the common conditions that are associated with the low vision population.
Have an understanding and ability to apply low vision rehabilitation strategies of magnification, lighting, glare control, contrast, and sensory substitution, organization, and memory.
Experience working with individuals with visual impairment.
Required Skills/Abilities:
Excellent oral and written communication skills.
Proficient with Microsoft Office, including Word, Excel, and Outlook.
Ability to travel throughout the service area in southwestern Ohio and northern Kentucky.
