Work Schedule: 8 Hr (8:30:00 AM - 5:00:00 PM)

Summary:

The Manager, Care Management Registered Nurse is responsible for managing and leading a multidisciplinary care team composed of social workers, nurses, dieticians and care navigators to provide effective, efficient care coordination for all Rush Health value-based clinical programs including but not limited to Rush Health's Centers of Excellence program and the CMS Oncology Care Model. Leads the day-to-day activities of the care managers, including training, competency review, mentoring and problem solving. Ensures care managers are able to provide high quality, holistic healthcare to patients with multiple chronic conditions, social risk factors, and transition of care needs through a patient-centered approach. Collaborates with the Director of Clinical Outcomes to identify improvement opportunities, create efficient workflows and processes and leverage resources appropriately. Works with care managers and Rush Health members to identify ways to improve integration of care management services into the care teams. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Required Job Qualifications:

•Bachelor's degree in Nursing from an accredited college or university.

• Current State of Illinois Registered Nurse licensure.

•Three years of care management work experience for a health plan or multi-specialty medical group.

•Two years of supervisory work experience with a multi-disciplinary team.

•A basic understanding of risk-based agreements (ACOs, capitation).

•Proficiency using an electronic medical record.

•Strong written and verbal communication skills: proven presentation skills needed to interact with both internal and external stakeholders.

•Exceptional interpersonal skills: customer-service focused and able to interact effectively and professionally with all levels of management, employees and referral sources.

•Team building skills and flexibility.

•Leadership skills, ability to perform tasks independently, manage priorities, collaborate with others, and achieve outcomes.

•Critical thinking and analytical skills and competency related to information systems and windows technology.

Preferred Job Qualifications:

•Master's Degree

•Proficiency using EPIC.

•Previous experience in an ambulatory setting.

Responsibilities:

  1. Functions as an expert clinical practitioner within the team and serves as a resource for the care team.

  2. Shares his/her professional expertise and serves as a resource to manage clinically complex cases at the appropriate level of care.

  3. With the Director of Clinical Outcomes, assures care mangers are meeting contract requirements for care management services

  4. Creates and updates evidence-based workflows, policies and procedures designed to reduce total cost of care, improve quality, and patient experience.

  5. Provides leadership within departmental projects related to the Care Management role. Actively supports institutional initiatives utilizing nursing clinical and CM expertise.

  6. Hires, trains and supervises care management staff. Includes conducting chart audits to ensure adherence to outlined processes.

  7. Lead weekly team meetings and case reviews to promote collaborative planning and advancement of the plan of care.

  8. Develops ongoing competency trainings for care management staff.

  9. Works with IS department to create and optimize care management tools available in the EMR.

  10. Collaborates with Rush Health member organizations to successfully integrate Rush Health care managers into existing care teams.

  11. Use existing data to monitor and analyze effectiveness of care management interventions.

  12. Update interventions when needed.
