Community Support Guide - Part Time- Lewis County
Our organization is dedicated to empowering individuals with intellectual disabilities to achieve their fullest potential. We work hand in hand with these individuals to identify their aspirations and assist them in realizing their goals, ranging from volunteering at a local food bank to acquiring skills such as ordering food at a restaurant. At our organization, no goal is too small or too big. As a Community Support Guide, you will be responsible for transporting clients to and from community locations to work on their specific goals. You will spend at least two hours with each client, helping them achieve their aims before returning them home. It's worth noting that remuneration is applicable from the moment you pick up the client until the time you drop them off at their destination. Additionally, mileage reimbursements are only applicable for the duration when the client is present in your vehicle.
QUALIFICATIONS AND EDUCATION QUALIFICATION:
Educational Requirements:
High School Diploma or GED
Bachelor's, Master's or Doctoral degree in:
*Psychology Sociology *Social Work Education *Certified Life Skills Coach
Work Experience:
At least one year experience with working with individuals with intellectual disabilities is required.
License/Certification: *Must have a vehicle up to date vehicle insurance.

ESSENTIAL FUNCTIONS OF THE JOB INCLUDE BUT NOT LIMITED TO:
*Provide assistance in accomplishing goals stated in the service plan developed by the Clients Case Manager, the Client and their family. *Develop, with the family, strategies to connect the Client with community resources, based on their interests and needs. *Bring the Client together with local community members, who are not paid for their involvement with the family, for mutual benefits and activities. *Gather information needed to enhance the Client participation in their community, providing Client with information that is written format, such as literature and pamphlets, for future reference. *Identify service goals. *Summary of the Client's progress towards achieving the services goals and objectives in measurable terms. *Provide progress notes after each session and quarterly.
