Job Description:

Coordinates the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing and evaluating. Provides leadership for the Health Care team by working cooperatively with other team members in maintaining standards of professional nursing practice in the Home Care setting.

Minimum or Preferred Qualifications:

AAS in Nursing required; BSN preferred. 1 year of psychiatric experience as a RN with good working knowledge of psychiatric diagnosis, psychiatric medication and treatment (within the last 3 years) required. 1 year of Medical/Surgical experience as a RN required. RN license, NYS Driver's license, and current APNA Transitions in Practice Certificate Required upon hire.

8:00AM-4:30PM M-F With Week-end, Holiday and on Call Rotation.

Salary Range:

$40.80-$50.90 per hour Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
