Long Island Select Healthcare Inc. (LISH) is a comprehensive network of community health centers under a Federally Qualified Health Center (FQHC) model. We provide Article 16 and 28 clinic services at 3 locations across Suffolk County, New York.

LISH is currently searching for a Full-Time LPN who will work under the direct supervision of the Lead RN to support LISH's providers in the provision of patient care. We are seeking compassionate, energetic, organized, and flexible individuals who will have the opportunity to work with experienced clinical staff.

Applicant must be able to provide a FLEXIBLE schedule while maintaining full-time hours. Our staffing needs may require either a four or five day work week, or may require applicant to travel to different sites depending on scheduling needs (i.e. vacations, sick coverage, changes in provider schedules requiring staffing adjustments).

General duties include:

  • Records vital signs

  • Performs EKGs, phlebotomy, testing, administering injections

  • Performs duties as assigned by the Chief Medical Officer and/or Clinic Director to support and ensure general medical operation of LISH.

  • This position will work closely with LISH's Chief Medical Officer & Nurse Manager

  • Assist with procedures and proper documentation utilizing an EMR (eClinicalWorks)

  • Involvement with LISH's PCMH Initiative, Chronic Care Management Program, Quality Initiatives, Lean processes, and more!

  • Please note that there will be extended hours on certain days.

    Requirements:

  • NYS Licensed LPN or MA certificate

  • Minimum of two years direct patient care experience, preferably in a fast paced ambulatory care setting/clinic.

  • Scheduling Software and EMR experience, eClinicalworks preferred

  • BLS certified

  • Bilingual in English and Spanish is a plus

  • Customer friendly attitude

  • Ability and Means to travel between sites

    Benefits

  • $25.58 - $31.17/hr, dependent upon experience as an LPN.
