Case Manager Aide
EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Case Manager Aide to work part time for its Community Guardianship Program.
The Case Manager is a Part-Time position paying $18-20 per hour at up to 18 hours per week with flexible hours 3 days a week. The office is located in Central Islip, NY. The role reports to the Program Director.
Primary Purpose of Job:
The Case Manager Aide provides administrative support to Case Managers and the Program Director, facilitating case management services for incapacitated adults.
Principal Duties & Responsibilities:
Assist case managers with case management services, including applying for and maintaining essential government benefits.
Maintain the case management database.
Perform clerical tasks, such as answering phones, managing calendars, and sorting mail.
Assist in preparing required reports for clients, including Initial Reports, Interim Reports, Annual Reports, and court updates, ensuring compliance with legal and program standards.
Execute other job-related duties as directed by the supervisor.
Collaborate closely with case managers.
Report to the supervisor and communicate effectively to ensure smooth daily operations.
Knowledge, Skills, and Abilities Required:
High School Diploma or equivalent required.
Relevant work experience in social services, case management, or a related field.
Strong proficiency in Microsoft Office, databases, and innovative tech solutions for case management.
Excellent communication skills (both verbal and written) with the ability to connect with diverse communities.
