Advanced Practice Provider Division: Health Reports to: Assistant Medical Director/ Health Director Worksite: Varied Clinic Sites Work Hours: M-F, 8:00am-4:30pm LLBO Status: Non-Exempt Safety Sensitive: Yes License/Certification Requirements: Valid MN Drivers License, current insurance or be insurable. Minnesota State License of Nurse Practitioner (NP) or Physician Assistant (PA-C). National Board Certification. Summary: Provide primary health care services within the Tribal operated, community-based clinics. Supervisory Duties: None. Duties/Responsibilities: Provide high quality health care services within scope of practice designated by the specific certification held by the provider including the following: Accessible and available health services. Education of and effective communication with those serve, concerning the diagnosis and treatment of their medical conditions, appropriate preventative measures and used of the health care system. Treatment that is consistent with clinical impressions and working diagnoses. Continuity of care. Complete appropriate, accurate and timely medical record entries. Adequate and appropriate transfer of information when clients are referred to another health care provider. Assess real or potential health hazards and current health status, including analysis of health behavior related to lifestyle and culture. Devise, implement and evaluate plans of care utilizing sound clinical judgements based on assessment of the physical, psycho-clinical judgements based on assessment of the physical, psychological, emotional, societal and environmental needs of the patients. Assist individuals to assume responsibility for the prevention of illness and promotion, maintenance and restoration of health. Consult, refer and collaborate with other disciplines involved in the delivery of total client care. Utilize the resources of the reservation community. Promote collegial relationships with other clinical staff and providers in the community. Demonstrate evidence of continuing professional growth by involvement in peer review and the review of protocols and procedures. Participate in continuing education as required for licensure. Demonstrate awareness of issues relating to nursing practice and health care. Participate in community education and other professional activities. Utilize the Electronic Health Record system, including: coding, referral tabs and templates. Maintain active medical staff membership/ privileges at Cass Lake Indian Hospital IHS, and comply with bylaws, rules, regulation and policies thereof. Attend and or conduct all mandatory meeting and trainings as required. Maintain confidentiality and professional ethics at all times. Travel in and out of the state on a potential regular basis. Adhere to policies governing staff conduct. All other related duties as assigned.
