[The nursecoordinates the physical health and wellness care of the residents and young children of the LifeHouse Crisis Maternity Home and other Maternal and Family Programs services (i.e., AfterCare program, Dedicated Dads, etc.). What youll do:

• Educate and facilitate health and wellness classes using evidence-based curriculum.
• Facilitate individual client meetings for the purpose of educating on prenatal and postpartum care.
• Accompany resident/client to medical appointments and communicate with health care providers to assist with achieving overall health goals.
• Monitor physical appointments for residents, mother, infant and/or children.
• Complete prenatal and postpartum depression screenings with intentions of following up with each resident/client.
• Provide parent education to resident/client families, including Dedicated Dads.
• Attending community networking meetings.
• Performs other related duties as assigned.
