Description

Reports to:Medical Director / Practice Manager

Rate of Pay:$70,000 - $90,000 for full-time status; part-time or per diem shall be prorated based on scheduled hours

Work Schedule:Flexible within Monday - Friday daytime office hours; no evenings, weekends, or major holidays; no on-call; full-time, part-time, or per diem schedules available

Position Summary:Provides counseling services to health center patients in compliance with standard practices.

Primary Responsibilities:

  • Perform psycho/social assessment.

  • Develop a therapeutic counseling regimen to meet the needs of the individual based upon assessment and input from caregivers/family.

  • Coordinate treatment plan with the psychiatrist, referring individuals as necessary for psychiatric consult and prescription.

  • Provide ongoing counseling as indicated and follow up if psychiatric consultation is made.

  • Maintain patient records per agency policies and procedures.

  • Provide ongoing supervision of MSWs as necessary.

  • Perform other duties as requested by the supervisor.

Requirements

  • Master’s Degree in Social Work

  • Current, registered NYS LCSW license

  • Experience with electronic medical records is preferred.

  • Strong organizational and planning skills.

  • Ability to maintain good working relationships with others.

  • Exhibits a high level of professionalism and confidentiality when dealing with issues.

  • Ability to lift and carry up to 25 lbs.

  • Ability to sit for long periods.

  • Ability to tolerate exposure to computer monitor/screen.

  • Ability to tolerate the extended use of a computer keyboard.
