FREMONT REGIONAL HOSPICE Certified Nurse Aide in CANON CITY, Colorado

POSITION: Certified Nurse Aide

The Certified Nurse Aide (CNA) is a member of the Interdisciplinary Team (IDT) who works under the supervision of the Registered Nurse or therapist and performs various services for a patient as necessary to meet the personal needs and to promote comfort. The CNA is responsible for observing the patient, reporting these observations, and documenting observations and care performed.

DUTIES: Preforming personal care needs for the patients and attending to their request promptly. Updating the case manager of any changes and documenting in accordance with federal, state, and organizational guidelines, and participating in quality improvement projects. Taking and recording oral, rectal, and auxiliary temperatures, pulse, respirations and blood pressures when ordered. Providing proper care and observation of patient s skin to prevent breakdown of tissue. Performs simple dressing changes under the supervision of the RN as assigned. Assists in ambulation and exercise as instructed by RN or therapist. Offering and assisting with bedpans and urinals. Providing assistance, as assigned, with light laundry needs. Performing Range of Motion (ROM) and other simple procedures as an extensional therapy service as ordered. (Within the limitations of the aide.) Maintain patient s living area as assigned. Adherence to the organization s documentation and care procedures, and standards of personal and professional conduct. Maintains confidentiality of all patient related matters. Maintains accurate and thorough records, documenting care provided in a timely manner. (Preferably within 24 hours of visit) Participates in team conferences. Must attend and complete the next scheduled Hospice Education and Training Program.

PHYSICAL REQUIREMENTS: This job requires Hospice personnel to lift/carry, squat, kneel, climb, bend, twist, and reach. Please refer to the company s job description for more information.

PREFERRED SKILLS: Possesses and maintains good physical stamina and mental health. Has ability to read and follow written instructions and document care given. Understands Hospice philosophy, focuses on comfort, and provides specialized care to the terminally ill. Self-directing with the ability to work with little direct supervision. Secure with issues of death/dying. Has empathy for the needs of the ill, injured, frail, and the impaired. Demonstrates tact, patience and good personal hygiene.

REQUIREMENTS: (18) eighteen years of age High school graduate or equivalent Minimum education requirements: Colorado State CNA license One (1) years experience as a Hospice CNA preferred Must possess a valid driver s license by the state of Colorado

OTHER REQUIREMENTS: All offers of employment are conditional pending clearance of a background screening, drug screen, TB test and verification of your ability to work in the United States.

CONTINUING EDUCATION REQUIREMENTS: The agency personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, Hospice personnel are expected to accept personal responsibility for other educational activities to enhance job relates skills and abilities. All personnel must attend mandatory educational programs.

WAGE: $17/ Hour - depending on experience

SHIFTS: Monday Saturday (8:00am 4:00pm) Position is Part-time with the possibility of becoming Full-time.

JOB SITE: Canon City, CO
