Department Description:
CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
Job Purpose:
The Behavioral Health Clinician is a member of the PACE Interdisciplinary Team who provides a range of clinical psychosocial services to participants at the PACE clinic and within the community. The Behavioral Health Clinician, along with other members of the Interdisciplinary Team, work to carry out the individual comprehensive care plans of our program participants. Working within the PACE model of care, they provide clinic-based and in-home clinical case management, assessment, crisis intervention, supportive counseling, and community resource coordination. They are expected to complete initial, semi-annual, and annual assessments, as well as provide supportive visits and follow-ups with family and collateral contacts, including within participants’ homes.
Qualifications:
Current Massachusetts LMHC, LCSW, or LISCW license in good standing
Experience with such therapies such as Motivational Interviewing, Cognitive Behavioral Therapy, Dialectical Behavioral Therapy or Mindfulness Based therapy
Minimum one year experience providing clinical social work to older adult population preferably in a community or medical setting
Computer literacy; experience with Epic beneficial
Reliable transportation to be able to attend home visits in a timely manner
Ability to work collaboratively in a variety of settings including Assisted Livings, Nursing Homes and Community Housing.
Experience working with individuals with dementia and their families/natural supports beneficial
Commitment to equitable and culturally appropriate care for a wide range of diverse populations, including (but not limited to) communities of color, LGBTQ communities, non-English speaking populations, people with histories of incarceration, people with substance use disorders and behavioral health challenges
Fluent in Spanish, Haitian Creole or Portuguese a plus
Interest in working in a collaborative, team environment.
