Your Specific Responsibilities:

Provides health care services in a clinical setting. Services include family planning, disease prevention (STD, HIV, TB), immunizations, and child health. Records patient's medical information and vital signs and gives prescribed medications per protocol. Prepares rooms, sterile instruments, equipment, and supplies, and assists provider as needed. Case manages referrals. Administers and counsels on immunizations according to procedure and in a timely sequence according to protocol and ACIP recommendations. Orders, conducts, interprets, and/or evaluates diagnostic tests to identify and assess patient’s condition. Performs venipuncture, obtains specimens, and sends them to laboratory to be analyzed.

Assists in the HIV Ryan White program. Responsible for client eligibility and ADAP and enrollment through the PROVIDE system annually and following up on client’s medications. Ensures clients compliance with requirements of the program. Responsible for gathering Ryan White data on types and costs of services performed throughout the month. Prepares a summary report for submittal to the HIV/TB supervisor for review and action.

Responsible for managing the clinical vaccine inventory at both sites. This includes ordering, temperature monitoring and weekly inventory counts.

Assists in the TB program. Provides health care services in the TB program by interviewing patients, making field visits to patient’s home, place of employment, and/or recreational area. Serves as case manager consulting with providers regarding client’s illness. Follows up to ensure medication compliance via direct observation therapy (DOT).

Serves as a back up to the Special Needs registry coordinator by reviewing, assessing, and evaluating applications for Shelter and transportation needs. Assigns to appropriate shelter (Hospital, ALF, Nursing Home, or Special Needs Shelter/General Shelter). Ensure forms are entered into the database. Communicates with applicants to clarify, obtain additional information, and assign to specific shelters based on medical/ physical needs. Coordinates with appropriate medical staff to assign complex cases to appropriate level of sheltering based on scopes of care. Assists with completion of monthly audit of internal Sumter County Registry. Conducts annual recertification of registered clients. Meets with the Special Needs Shelter teams and works with Sumter County’s EOC concerning special needs. Coordinates with Red Cross, Transportation, and other agencies as necessary to meet the safety needs of the registrants.

Required Knowledge, Skills, and Abilities:

  • Knowledge of nursing principles and techniques.

  • Knowledge of vaccine schedule, types and routes of administration

  • Ability to work effectively with diverse populations, communities, and organizations.

  • Ability to use the nursing process

  • Ability to follow and work independently using the Standard of care nursing protocols.

  • Ability to administer injectable medications and vaccines

  • Ability to educate staff, clients, organizations, and communities on importance of immunizations and health.

  • Ability to prioritize, organizes, and schedule work assignments to meet deadlines.

  • Ability to communicate effectively verbally and in writing.

  • Ability to utilize a computer for data entry and health record documentation.

  • Ability to maintain records and reports according to program standards.

    Qualifications:

    Minimum-

    Registered nurse licensure in the state of Florida or a multistate license CPR certification

    Preferred-

    At least one-year recent experience working as a RN

    At least one or more years of experience working in a public health environment

    Experience working with HIV/TB clints in a clinical setting

    Experience with Health Management System (HMS).

    Experience in documenting and retrieving clinical information in the computer
