Home Care Assistance of Greater Burlington Client Care Manager in Burlington, Vermont

Home Care Assistance of Greater Burlington is seeking a Client Care Manager to join us in our mission to Change the Way the World Ages. As a Client Care Manager, you would assist clients and their families with decisions related to aging, available services and long-term care planning. This individual will assess each family situation by evaluating the client’s functionality, health, emotional state and living environment, as well as their financial needs, in an effort to enhance their overall quality of life. Care Managers are also responsible for outreach work to attract new client referrals.

Position Essential Duties & Responsibilities:

  • Uses professional care management skills to independently assess and reassess client status, including physical and mental health, social resources, environmental safety, economic resources and functional ability.

  • Drafts, implements and revises the client Plan of Care to ensure appropriateness, effectiveness, efficiency and client satisfaction.

  • Collaborates with families and other service providers to facilitate care coordination and information sharing.

  • Provides supportive counseling to and conflict resolution with clients and family members; serves as an advocate for the well-being of the client.

  • Makes referrals to appropriate community agencies such as crisis intervention, legal advocacy and counseling agencies for medical, mental, physical, financial, legal and social needs.

  • Assists in matching and scheduling of caregivers for client needs.

  • Committed to eliminating open shifts including a willingness to provide fill-in caregiving work to assure client satisfaction.

  • Actively follow-up with potential and active contacts in our community to network and grow the business.

  • Proactively networks with and referral markets to the local peer eldercare professionals.

  • Provide portions of our caregiver training.

Position Knowledge and Skills Requirements:

  • Bachelor’s Degree in Gerontology, Psychology, Nursing, Health and Human Services, or another related field preferred. Licensure in Clinical Social Work highly desired

  • A minimum of 2 years’ experience in geriatrics and/or care management required; other professional experience may substitute

  • RN, PT, PTA, OT, LiCSW, or Care Management Certification (CCM or CMC) required

  • Previous work in the home care industry and/or caregiving a plus

  • Experience in sales a plus

  • Ability to interface effectively and professionally with client committee, including family, physicians, attorneys, etc.

  • Ability to work in a team environment required

  • Excellent written and verbal communication skills required

  • PC/Mac literacy required

  • Valid driver’s license and proof of current auto insurance required

Position Benefits:

  • $60,000 to 80,000 base salary

  • Plus quarterly bonus plan up to 20% of base.

  • Additional off-hour caregiver pay for fill-in shifts
