Responsibilities:
Performs initial intake and therapy assessments based on the referral from the medical practitioner.
Updates evaluation each visit with a behavioral health note reflecting changes from visit to visit and the initial assessment.
Creates a therapeutic treatment plan clearly identifying patient’s goals for treatment.
Addresses treatment plan issues and any barriers to achieving goals on treatment, in addition to any new concerns that may arise between visits.
Defers to different practitioners within Neighborhood Health Center (NHC) if an issue arises that is outside of his/her scope of practice.
Makes appropriate referrals in the community for any services not able to provide at NHC.
Reiterates patient’s need to adhere to practitioner’s orders regarding non-behavioral health issues, in order to optimize best possible health.
Demonstrates an awareness and sensitivity of all factors affecting an individual patient. These are including, but not limited to, race, gender, sexual orientation, religion and culture.
Addresses behavioral health issues identified on the Problem List and keeps list accurate and up-to-date.
Performs all documentation and coordination with the medical practitioner and each chart will be signed off on by appropriate practitioner and administrator within 24 hours.
Appropriately codes each encounter to ensure appropriate billing measures.
LMSW submits chart to LCSW-R within 24 hours for review and sign off.
LMSW performs quarterly peer chart reviews and completes identified deadline.
LMSW acts as a patient advocate when appropriate and helps patient develop and acquire necessary life and coping skills.
LMSW facilitates Advanced Care Management Framework for assigned patients
LMSW maintains or provides information for patient registry to meet grant expectations
LMSW facilitates telehealth visits when appropriate and schedule with patients and providers
LMSW supports patient adherence to treatment between visit intervals to ensure safety and success
LMSW implements harm reduction strategies for SUD patients
LMSW works to enhance NHC and partner organization relationships
LMSW adheres to NASW code of ethics to ensure best quality of care for patients.
Adheres to universal precautions and infection control standards of practice within the clinical setting.
Maintains competency in organizational emergency protocols and response.
Adheres to HIPAA practice standards of patient confidentiality at all times including use of EHR.
Adheres to clinical standards of dress appropriate for health care professionals.
Other duties as assigned.
Compensation: LMSW - $62,000-$66,000; LCSW - $67,000-$72,000 annual salary (based on a full-time, 40 hour work week)
Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
Requirements:
Must possess a Master’s Degree in Social Work and a strong desire to work with an urban population and possess a thorough knowledge of health issues affecting the population throughout the lifecycle. Must be able to read, write and speak the English language. A second language of Spanish preferred.
