Job Title: Care Manager I

Department: Child and Adolescent Care Coordination

Reports to: Program Director, Supervisor

Location: Hybrid (field work required - through Bronx, Manhattan, Queens, and Brooklyn)

Schedule: Monday- Friday - 9-5; some evening hours may be required to fit the needs of meeting with clients

Salary Grade: $28.57 - $29.80

Position Overview:

Vibrant (formerly the Mental Health Association of New York City) is currently seeking dynamic youth health advocates to join our Child & Adolescent Health Home Care Coordination department. The aim of care coordination is to build well-working teams by ensuring that all parties are informed and effectively communicating, while also finding and linking services that could further help a young person to achieve the best possible outcomes. Our approach to Care Coordination is oriented toward wellness, is family-focused and supports a strength based, person-centered system of care. Care Managers will ensure that the young people receive timely access to services, as well as, coordination and management of care across all systems (education, mental health, juvenile justice, child to adult). Care Managers will also track clinical and functional outcomes using standardized tools that are validated for the screening and assessment of children.

Duties/Responsibilities:

The care managers are field-based and are provided a laptop and smartphone to ensure functionality. Care managers are currently expected to serve Manhattan, the Bronx, Brooklyn, and Queens.

Comprehensive Care Management

● Performs enrollment activities, such as providing families with objective and goals of Health Home enrollment and obtains necessary documents (consents, demographic information, etc.)

● Manage a caseload of 20 clients aged 0-21 and facilitates monthly sessions via a hybrid model: in-person at residents home, office-based, community based, school based etc. Clients reside within Bronx, Queens, Brooklyn and Manhattan.

● Develops and monitors care plans through the use of assessment tools and other medical/behavioral health documentation (progress notes) to determine health needs of children/youth enrolled in the Health Home

● Completes CANS-NY assessment for children/youth on their caseload

● Responsible for maintaining CANS-NY certification on a yearly basis

● Adheres to DOH guideline of timely documentation to be completed by 48 hours of service

● Performs monthly required contacts based on level of care/need

● Completes necessary documents for monthly care management billing to Health Home/Managed Care Organization

● Completes all trainings related to Compliance, Quality Assurance, Care Coordination as needed on a yearly basis

Care Coordination and Health Promotion:

● Coordinates between Vibrant, caregivers, and community providers to deliver health care services including: primary and specialized care, dental care, optical, behavioral health, and pharmacologic services

● Supports medical/behavioral health treatment recommendations

● Conducts appropriate health assessments of each child/youth on caseload,

● Monitors change in client’s condition/functioning that necessitates treatment and service changes

● Ensures timely access to care. Supports continuity of care

● Shares the care plan across a multidisciplinary team (community-based and other social support services, as well as healthcare services, that respond to the individual´s needs, preferences)and participates in case conferences with clients and stakeholders

● Solicits input from caregivers and providers as to the success of the care plan

● Identification of available community-based resources and actively manages appropriate referrals, access, engagement, follow-up and coordination of services.

● Work with socially, economically, culturally, and ethnically diverse groups in a mutually respectful manner using a strength-based and flexible approach

● Other duties as assigned

● Use of Health Information Technology (HIT) to Link Services

● Utilization of electronic health records and platforms to ensure comprehensive documentation.

Required Skills/Abilities:

● Relevant experience or knowledge and experience in serving children and families in child welfare, developmental disabilities, mental health, school, healthcare and / or other systems as well as those receiving preventive services

● Some experience or knowledge of providing service coordination and information, linkages, and referrals for community-based services

● Knowledge of the provision of health care in a variety of settings

● Ability to work directly with a diverse population consisting of Severely Emotionally Disturbed (SED), Medically Fragile, Division of Juvenile Justice, Developmentally Disabled (DD)

● Detail oriented

Required Certifications:

Child and Adolescent Needs and Strengths Assessment (CANS)

Candidates must complete training and pass certification for CANS 0-5 and 6-21 within probationary time in order to be cleared to begin building a caseload.

Required Qualifications:

● Bachelor Degree

● 1-2 years of relevant experience performing similar responsibilities

● Bilingual candidates are encouraged to apply

● Openness to travel within Bronx, Manhattan, Queens, and Brooklyn

● Knowledge of Microsoft Office products (Word, Excel, Powerpoint) and Google drive

● Knowledge or ability to work within multiple data systems

● Strong written and verbal communication skills

● Knowledge of crisis management

Physical and Sensory Requirements:

Ability to work collaboratively with staff, residents, service providers, families, and others

Ability to respond to telephones and other auditory stimulation

Travel:

This is a field based position. Traveling is required to complete monthly visits to your caseload.
