Brockton Neighborhood Health Center MD -- Endocrinologist in Brockton, Massachusetts

POSITION SUMMARY: The Endocrinologist will treat patients who have medical conditions that affect the endocrine glands and the hormones produced by them to include common conditions such as thyroid diseases, diabetes, infertility, hypertension, cholesterol disorders, osteoporosis and cancers in the endocrine glands. Provide medical care and treatment to Brockton Neighborhood Health Center patients in accordance with professional standards and established Brockton Neighborhood Health Center policies and procedures. PRINCIPAL DUTIES AND RESPONSIBILITIES: 1. Examine patients with suspected endocrine issues and review previous physicians notes and test results, conducting a physical exam, running lab tests, and developing a course of treatment. 2. Provide patients with information on diet, hygiene and other home treatments that will help manage their condition. 3. Counsels patients on subjects such as diet, hygiene and other areas of preventative maintenance for problems such as hypertension and cholesterol levels. 4. Assesses, plans, documents in EHR, and evaluates patient needs and treatments. Ensures appropriate follow up. 5. Maintains a practice that is culturally and linguistically sensitive, using interpreters and other assistance as appropriate. 6. Designs a plan of care based on assessment of physical, emotional, and social needs and in collaboration with the patient and, if appropriate, the family. 7. Refers patients, as appropriate, to specialists and other practitioners and facilities. 8. Reviews patient charts and ensures that appropriate treatment plans are developed and followed. 9. Demonstrates sensitivity and responds to verbal and non-verbal communications of patients, families and co-workers. 10. Maintains patients right to privacy, assuring dignity in the provision of care and is discreet in all communications, written and oral. 11. Consistently utilizes good judgment in the resolution of problems, clinical and non-clinical. 12. Collaboratively works with other caregivers regarding patient education and rehabilitation including promotion of mental and physical health. 13. Maintains collaborative relationship with other physicians, administration, and other clinical staff. Furnishes professional guidance and assistance to colleagues and other staff. Utilizes skills and expertise of other disciplines to ensure comprehensive care for all patients. 14. Cooperates with efforts of the Patient Care Committee to monitor and improve the quality of patient care provided. 15. Visit expectation for half-time physician is at least 2100 patient visits per year, 4200 for full-time. Education Requirement: MD plus 3-year residency internal medicine plus infectious disease fellowship
