Your Role:

  • Schedule: Monday through Friday, full-time days

  • Key Responsibilities:

  • Align all activities with the vision of Elara Caring’s leadership.

  • Assess hospice patients and families to identify their needs.

  • Manage a caseload, including planning, coordinating, and evaluating treatment plans.

  • Conduct initial and follow-up patient evaluations.

  • Communicate significant findings and changes to the Clinical Supervisor and other relevant personnel.

  • Revise care plans as needed and provide counseling and instruction to patients and families.

  • Teach hospice philosophy and pain management to staff in various care settings.

  • Supervise home health aides and Licensed Practical Nurses/Licensed Vocational Nurses.

  • Provide support at the time of death and perform bereavement assessments.

  • Document medication regimens and maintain confidentiality in compliance with HIPAA.

Requirements:

  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing

  • Current State Licensee as a Registered Nurse RN

  • 1 year of experience in a clinical care setting

  • Experience in hospice or home health environment is preferred

  • Nursing skills as defined by accepted nursing standards

  • Ability to handle multiple priorities, documentation requirements, and deadlines

  • Reliable transportation to perform job tasks

  • Preferred to live in the service area (less drive time for you!)
