Work Schedule: Two 7:00 PM to 7:00 AM shifts & one 10:00 AM-10:00 PM shift.

Summary:

Transfer Center Nurse is a fully competent practitioner of nursing. The incumbent is responsible for the practice of nursing according to the Rules and Regulations defined in the State of Maine Nurse Practice Act, Northern Light Health Standards of Care/Practice, policies, and procedures. Utilizing screening guidelines established by the Medical Director of the Integrated Transfer Center, the Transfer Center Nurse is responsible for obtaining clinical presentation data and coordinating the appropriate resources needed to care for the patient. The incumbent must have the ability to identify the acuity and gravity of the patient’s condition, comprehend complex clinical conditions including Northern Light Health’s capability and capacity of services and other logistical strategies for transfer/transport medicine. The incumbent performs duties and tasks in accordance with performance standards established for the job which includes the utilization of screening guidelines to connect to the appropriate providers and facilities for services, working with providers to achieve a discussion of acceptance and the most appropriate level of care for bed placement for the patient, complete and accurate patient/transfer information, obtaining clinical updates on patients waiting in the region in addition to the coordination of bed availability Northern Light Health facilities.. The incumbent is also responsible for managing bed placement for EMMC and maintaining a working knowledge of current Transfer Center/Bed Management policies and procedures for all member organizations.

Responsibilities:

Essential Functions

  • Collaborates with Patient Flow Manager and House Managers on flow prioritization.

  • Provides clinical guidance to Transfer Center Operations Specialist (TCOS) colleagues in both the bed management and Transfer center role.

  • Facilitate training/orientation of new employees.

  • Assumes responsibility for Northern Light Health required continued education and own professional growth.

  • Attends weekly operational meetings or reviews minutes prior to next shift.

  • Performs other duties as assigned or required.

Patient Safety

  • Maintains a safe and therapeutic environment by understanding and complying with hospital policies/procedures and national patient safety goals and identifying & communicating workarounds that have a potential impact on patient safety.

  • Demonstrates knowledge and skills in location, operation, care and troubleshooting of equipment independently.

Clinical Assessment

  • Works with providers to achieve discussion and/or acceptance and the most appropriate bed placement for patients.

  • Interprets and responds to abnormal diagnostic data.

  • Anticipates potential emergency situations and intervenes safely with assistance if needed.

  • Utilizes the screening guidelines to connect the appropriate providers.

  • Obtains clinical updates on patients awaiting placement in the region.

  • Maintains a working knowledge of current Transfer Center/Bed Management policies and procedures.

  • Effectively communicates needed information to Providers.

  • Provides prompt, effective service to internal and external customers.

Other Information:

  • Additional education and experience may be required according to work location.

  • Experience with EHR and supporting Technolgy required.

  • Clinical skills per the Maine State Board of Nursing Scope of the Practice for RN's required.

Education

  • Required Associate's DegreeBSN preferred. ED or ICU experience preferred.
