The summer internship will be supporting CCA’s work with Senior Care Options (SCO) delegated primary care sites and One Care (ICO) Health Home sites. CCA contracts with and works very closely with five delegated primary care sites in Eastern Massachusetts that have demonstrated outstanding commitment to delivering patient-centered care to seniors 65-years-old and up in underserved communities and with eight behavioral health and primary care organizations to MassHealth recipients 18- to 64-years-old. These sites have extensive experience in serving a frail elder population and adult members with serious mental health and medical needs. The resources and models of care of these sites are highly aligned with the needs of our members. CCA has delegated the care management of SCO and ICO members to these sites so that their care managers work together with providers in coordinating social supports with health delivery and extending primary care into the home setting.
Reporting to the SCO Delegated Sites Program Analyst, the Provider Partnerships and Performance Intern is a member of the Delegation Partnerships and Performance Team. The intern will undertake a variety of administrative tasks as well as assist both SCO Delegated Sites and Health Homes teams in daily management of key operational accuracies to include reporting, communications, and the onboarding and offboarding functions for new external partner employees. They will also collaborate with cross-functional departments such as Business Intelligence, IT, Clinical Informatics, Population Health, and Clinical Operations. This role is responsible for authentically engaging with colleagues and provider partners across CCA’s health system.
What You'll Be Doing:
Provide Project Management and Meeting Support to SCO Delegated Sites and Health Homes Team
Developing agendas, taking and disseminating notes, documenting action steps, communication and follow-up with members of group with the oversight of the Director
Support ad hoc meetings and trainings
Maintain outline of all reporting shared with provider partners
Develop CCA Resource Guides to promote strong communication and collaboration between CCA and provider partners
Assess state of CCA programs (Mobile Integrated Health, Hospital to Home, Complex Care Self-Management, PCA Assessment) utilization and codify processes. Work with Director to improve processes based on assessment.
Operations Management and Support
Review and develop Table of Contents for the Health Homes Extranet folders
Develop resource guides and organize team folders and team pages for SCO Delegated Sites and Health Homes
Update current SCO Delegated Sites Looker reference guides to reflect changes to existing dashboards and automated delivery services
Data project
Work with Provider Network Strategy Analytics Manager to analyze data related to SCO Delegated Sites and Health Homes
Project to be designed closer to start date
Disseminate information to SCO Delegated Sites and Health Homes
Assist in drafting communications of CCA updates to both teams
Ad hoc emergency communications on behalf of both teams
Strategic Work
- Independent Project focused on enhancing program operations
Additional Information
All internships will be paid standard and competitive rates for undergraduate and graduate levels ($23 and $25 per hour respectively). Internships are expected to be full-time through the duration of the program)
