Eliot Community Human Services HLS Stabilization Housing First Coordinator- Bilingual (Haitian Creole) in Boston, Massachusetts

Description

Eliot’s Homeless Services Division has received funding to open emergency family shelters and assist with stabilization initiatives with families experiencing street homelessness. This will include an array of onsite supportive services, including recovery coaching, MAT/Psychiatry, wrap-around case management & clinical coordination, medical consultation and direct interventions, and access to trauma-focused treatment modalities.

Eliot is seeking a creative/passionate Housing First Coordinator to provide onsite case management & support in a highly collaborative fashion with residents. Successful candidates will have unwavering compassion, as well as an open mind in terms of pathways to recovery/harm reduction with respect to substance use disorders. Working knowledge of area resources/providers, housing navigation, and tenancy preservation are a plus. Candidates with lived experience as strongly encouraged to apply.

Responsibilities:

  • Screen and assess anticipated needs of families assigned as they transition into housing in a collaborative fashion with the respective family, onsite case management, leadership and housing navigation team.

  • Flag high risk factors reported or observed and immediately report to the Director of Family Stabilization to formulate a plan of action to support the family.

  • Carries an assigned caseload of 25 families and collaboratively coordinates care and supports to address assessed needs and service delivery preferences.

  • Assists families in all aspects of maintaining tenancies including but not limited to: paying share of rent, primary and pediatric care, fuel assistance for utilities, food pantries, school enrollment, referral to local Family Resource Center/other relevant resources that serve new Immigrant population, US norms, child care and vocational support as needed.

  • Maintain weekly contact with assigned families with a minimum of 2 in person visits per month at the family’s residence.

  • Develop and modify individual client service plans based on changes in needs (e.g. behavioral health and/or medical).

  • Provide comprehensive service and care coordination through collateral contacts as indicated while leveraging pre and post tenancy support services within a Housing First framework.

  • Track USCIS immigration profiles and immigration/work permit status. Provides direct assistance and referral to appropriate resources in this realm.

  • Provides real-time guidance and support to families utilizing motivational interviewing and de-escalation techniques when relevant.

  • Collaborate closely with internal colleagues/leadership, vendors, volunteers, hotel staff, and community-based organizations.

  • Perform other job-related duties as assigned.

Qualifications :

  • Requires a Bachelor’s degree in social work, counseling, psychology, nursing or other closely related field– or 2 years case management or comparable experience with the homeless population and/or families.

  • Master’s degree in the healing arts is strongly preferred or Lived experience with at least 1-year experience with the individual and/or family homeless population will be considered

  • Ability to provide flexible services that are community based and focused on team work.

  • Must have a reliable vehicle and a valid drivers license on Day 1 of hire.

  • Bilingual language capacity (Spanish, Portuguese or Haitian Creole) is preferred.

  • Must demonstrate proven ability to leverage internal/external continuums of care and resources to meet identified needs of families served while maximizing their input and participation.

Schedule: Full Time, Monday-Friday, 9am-5pm

Non-Bilingual Annual Salary:

  • Bachelors Degree- $48,000 annually

  • Masters Degree- $50,000 annually

Bilingual Salary:

  • Bachelor's Degree- $50,880 annually

  • Master's Degree- $53,000 annually
